In order for this permit application review to continue, a detailed addendum and <br /> revised sheets for plans must be provided for the following deficiencies: <br /> 1. Page 3 of the Application for Underground Storage Tank Installation Permit does not <br /> specify a contractor. Specify a contractor and submit a copy of the contractor's <br /> technician certifications. Submit the updated Page 3 of the Application for Underground <br /> Storage Tank Installation Permit. <br /> 2. Page 4 of the Application for Underground Storage Tank Installation Permit indicates <br /> that Enhanced Leak Detection (ELD) must be performed prior to the final inspection. A <br /> separate permit application is required to install ELD. Submit the required application. <br /> 3. Page 6 section E of the Application for Underground Storage Tank Installation Permit <br /> does not have the box checked for Buoyancy Calculations. Check it. Submit corrected <br /> version. <br /> 4. Page 7 section G of the Application for Underground Storage Tank Installation Permit <br /> does not list the manufacturer and model of the overspill containment having minimum <br /> capacity of 5 gallons provided at each tank fill location. Fill in the blank. Submit corrected <br /> version. <br /> 5. Page 7 section G of the Application for Underground Storage Tank Installation Permit <br /> lists an OPW 61fSTOP-2000 as the overfill prevention at 95%. However, an <br /> 0PW61fST0P is approved for aboveground storage tanks. Choose equipment that is <br /> approved for underground storage tanks. Correct the discrepancies. Submit corrected <br /> version. <br /> 6. Page 7 section G of the Application for Underground Storage Tank Installation Permit <br /> indicates that an approved overfill prevention device must be provided at each tank fill <br /> location. Choose one of the following methods. You may refer to the State Water <br /> Resources Control Board Letter of Guidance 150-2. Submit corrected version. <br />
