0 0 <br /> • E4.0 SPILL INCIDENT REPORT <br /> The spill incident report will contain the following information: <br /> • Description of the spilled material including identity, quantity, and a copy of the <br /> waste manifest <br /> • Cause of the spill <br /> • Exact time and location of the spill <br /> • Containment procedures used <br /> • Description of the corrective actions implemented during the spill including the <br /> disposal of the cleanup residue <br /> • Summary of the communications between CKY and government officials other <br /> than the COR <br /> E5.0 DISCHARGE CONTROL <br /> • Contamination may be spread by an accidental discharge of contaminants due to runon <br /> and runoff. Potential discharge of contaminants due to runon and runoff will be diverted <br /> by surface grading, trenching, diking, or use of sandbags. CKY will perform excavation <br /> and backfilling in a manner and sequence that will minimize runon and runoff. The <br /> excavated contaminated materials, including excavated soil, will be covered with <br /> visqueen (heavy duty plastic sheathing) and enforced with sand bags at the end of each <br /> work day while construction is in progress. CKY will ensure that site activities are in <br /> compliance with all federal and State stormwater management regulations, including the <br /> National Pollutant Discharge Elimination System (NPDES) program requirements. <br /> The following procedures will be used to prevent discharging potentially contaminated <br /> materials: <br /> • Potential contaminated materials and other hazardous waste will be covered if not <br /> containerized. <br /> • Excavations will be diked and covered securely with visqueen after work hours if <br /> any precipitation is expected. <br /> • Rain-water that may be accumulated in the covered excavation will be pumped <br /> out before work resumes. <br /> • <br /> E-3 <br />
