0 0
• E4.0 SPILL INCIDENT REPORT
The spill incident report will contain the following information:
• Description of the spilled material including identity, quantity, and a copy of the
waste manifest
• Cause of the spill
• Exact time and location of the spill
• Containment procedures used
• Description of the corrective actions implemented during the spill including the
disposal of the cleanup residue
• Summary of the communications between CKY and government officials other
than the COR
E5.0 DISCHARGE CONTROL
• Contamination may be spread by an accidental discharge of contaminants due to runon
and runoff. Potential discharge of contaminants due to runon and runoff will be diverted
by surface grading, trenching, diking, or use of sandbags. CKY will perform excavation
and backfilling in a manner and sequence that will minimize runon and runoff. The
excavated contaminated materials, including excavated soil, will be covered with
visqueen (heavy duty plastic sheathing) and enforced with sand bags at the end of each
work day while construction is in progress. CKY will ensure that site activities are in
compliance with all federal and State stormwater management regulations, including the
National Pollutant Discharge Elimination System (NPDES) program requirements.
The following procedures will be used to prevent discharging potentially contaminated
materials:
• Potential contaminated materials and other hazardous waste will be covered if not
containerized.
• Excavations will be diked and covered securely with visqueen after work hours if
any precipitation is expected.
• Rain-water that may be accumulated in the covered excavation will be pumped
out before work resumes.
•
E-3
