0 • <br /> • Deliver signed Daily Quality Reports to COR the following day which cover the <br /> work performed the day before. <br /> Certified Industrial Hygienist (CIH) <br /> Mr. Gus Ballis, Project Health and Safety Officer, will act as the CIH for this contract. <br /> Mr. Ballis will review and approve all Safety and Health documents related to the contract <br /> and to specific delivery orders. Health and safety procedures in individual delivery orders <br /> are consistent with those in the contract. He will determine the level of personnel <br /> protection, the components of the air monitoring program, and the medical monitoring <br /> program for the site workers. He will review and approve any changes to these documents <br /> and programs, review all air and personnel monitoring results, and make <br /> recommendations regarding hazard reduction techniques. <br /> Site Safety and Health Officer <br /> Mr. Robert Castaneda is the Site Safety and Health Officer (SHO). He will be responsible <br /> for monitoring the field personnel for compliance with the requirements of the Site-specific <br /> Health and Safety Plan. Specifically, the SHO will perform the following tasks at the project <br /> site: <br /> • Conduct daily tailgate meetings, <br /> Maintain a record of all entries into the exclusion zone (if any), <br /> • Monitor personnel who enter and exit the site and all controlled access points, <br /> • Report any signs of fatigue, work-related stress, or chemical exposures to the PM <br /> immediately, <br /> • Inspect PPE and clothing to ensure that it is in good condition and is being stored and <br /> maintained properly, <br /> • Understand and communicate emergency procedures, evacuation routes, and other <br /> emergency information, <br /> • Coordinate upgrading and downgrading of PPE (after CIH approval), <br /> • Perform all air monitoring with approved instruments and equipment, <br /> • Inspect and maintain monitoring equipment including calibration checks, and <br /> • Complete the required forms and documentation. <br /> Daily safety reports will be prepared by the SHO during field activities, and turned in to the <br /> QCI for review. In the event of unusually high readings (readings that exceed levels <br /> specified in the Site-specific Health and Safety Plan), the SHO will notify the Project Health <br /> and Safety Officer, QCI and the PM, and take appropriate action. <br /> The SHO has the authority to stop work if site conditions differ dramatically from those <br /> anticipated in the Site-specific Safety and Health Plan, if unsafe conditions are present, if <br /> 1-3 <br />
