0 • <br /> • Quality Control Manager <br /> Mr. Mark Hallock is the QCM for this project. He will be responsible for the QC program <br /> for field activities, including inspections, testing, surveying, restoration, materials used, and <br /> submittals. The QCM will report to the Project Director. A letter authorizing the QCM and <br /> outlining his responsibilities is presented in Attachment 2. <br /> The QCM will also be responsible for ensuring that needed information regarding QA/QC <br /> issues is supplied to the COR. The QCM will also check to ensure that proposed <br /> materials and submittals meet the requirements of the contract specifications before these <br /> items are submitted to the COR for review. <br /> Other responsibilities of the QCM's will include acting as a liaison between the suppliers <br /> and the project, ensuring that all materials, equipment, personnel and procedures are in <br /> compliance with the terms of the contract and in the event of a discrepancy, all matters are <br /> immediately corrected. All reports prepared by or reviewed by the Quality Control <br /> Inspector (QCI) will be reviewed by the QCM prior to forwarding to the COR. The QCM is <br /> responsible for resolution of all conflicts and discrepancies. <br /> Quality Control Ins ep ctor <br /> Mr. Mike Bailey will act as the project QCI and report to the QCM. <br /> • The QCI will have the same responsibilities as the QCM in the field when delegated and <br /> will report to the QCM. <br /> The QCI will carry out the following tasks: <br /> • Complete and/or compile the Daily Quality Control reports, the Daily Chemical <br /> Quality Control Reports, <br /> • Preside over all 3-phase inspections and document results, <br /> • Complete the field logbook, <br /> • Complete non-routine occurrences reports, <br /> • Complete accident reports (ENG Form 3394), <br /> • Perform and/or compile QA/QC inspection reports of Geotechnical and Materials <br /> Testing, <br /> • Prepare survey summary sheets, <br /> • Complete inspection checklists, <br /> • Collect all information and documentation required for the completion of field reports <br /> and closure reports, <br /> • Maintain the photographic record of the work progress, <br /> • Coordinate with the laboratory, <br /> • Ensure submittal of and review of laboratory reports, and <br /> • Forward the reports to the QCM. <br /> 1-2 <br />
