Turner Road, Lodi, CA <br /> Dear Mr. Naidu: <br /> On April 29, 2011 you completed an Underground Storage Tank Inspection at the MCI, doing <br /> business as, Verizon Business facility located at 2500 W. Turner Road in Lodi, CA. As part of <br /> the inspection, you listed the following item that required immediate attention. Below is my <br /> response: <br /> UST Item #23c: Designated Monthly Inspection: The UST designated monthly inspection for <br /> August and September 2010 was not completed by Gary Duvall. Gary was the MCI, doing <br /> business as, Verizon Business designated operator for this location. Due to a confidential <br /> internal company human resources issue, Gary was placed on temporary leave in mid-August <br /> 2010. Since this was a confidential internal human resources issue, Gary was only allowed to <br /> speak with his manager and human resources. Following a company internal investigation, <br /> Gary was terminated from MCI, doing business as, Verizon Business on September 3, 2010. <br /> With that being the case, Gary was not able to complete his August and September 2010 <br /> inspections. EPIC Compliance was retained to perform the monthly DO inspections for MCI, <br /> doing business as Verizon Business. I have attached a letter from EPIC Compliance regarding <br /> the October 2010 and November 2010 DO monthly inspections. <br /> I have enclosed the Return to Compliance Certification Form.
