The <br /> decision was mutually made to start services in October. EPIC acknowledges that there was an <br /> internal communication error that resulted in this particular site in Lodi to not be included in the <br /> routing for October visits. This error was not discovered until November 3rd. <br /> The decision was made on the 5th of November to give this job to anew employee, Julian <br /> Sherrod. <br /> What has since been discovered is that Julian Sherrod was assigned by Case Gilson to conduct <br /> several Designated Operator visits during the time period of November 8th, 1 Ithor 12th with <br /> the site on West Turner Road notated as a priority. It seems that Julian never provided the site <br /> located at 2500 W Turner Road with a check-sheet during this time period nor was it found to be <br /> separately notated on his time card. We have searched thru our archives and have not located the <br /> November documentation and can only conclude that the job was assigned and not completed. <br /> Julian's tenure with the company did not exceed his probationary period and he has not been in <br /> EPIC's employ since the 19th of November. <br /> Respectfully, <br /> Deanna Dizmang <br /> EPIC Compliance Systems, Inc <br />
