The following are Repeat Class II Violations <br /> 104, 303,305, 999 <br /> 104. (Repeat) Current financial responsibility documents have not been submitted to the EHD. Current <br /> financial responsibility documents are required to be submitted annually. Immediately submit a copy of <br /> the financial responsibility documents to the EHD. Alternatively, log into CERS, upload the required <br /> information, and submit for review by the EHD. Complete by 7/23/2014. <br /> 303. (Repeat) The audible alarm on the monitoring panel did not come on when the UST annular and <br /> STP sump sensors were triggered; therefor the monitoring system was not working properly during <br /> regular operation as it was designed. UST System shall be operated and maintained properly to ensure <br /> continuous monitoring of the tank system as required by CCR 2630(d). Technician subsequently <br /> powered down then powered up (not a cold start) the monitoring panel and retested the sensors, and the <br /> audible alarm was triggered and passed. Violation corrected on site. (See Notes) <br /> 305. (Repeat) The Bravo box float and chain in the sump failed to stop the flow of product at the <br /> dispenser when tested. All monitoring equipment shall be maintained to activate an audible and visual <br /> alarm or stop the flow of product at the dispenser when it detects a leak. The service technician <br /> adjusted the chain and verified functionality. Violation corrected on site. <br /> 999. HSC 25404 (e)(4). (Repeat) Electronic submittal requirement on the California Environmental <br /> Reporting System (CERS) has not been met. Facility shall submit electronic information for hazardous <br /> materials management system, including hazardous waste, on the CERS website, which can be found <br /> on http:.//cers.calepa.ca.gov/. Immediately comply with the CERS electronic submittal requirement per <br /> this section by 7/23/2014.
