See attached'Continuation—Official Inspection Report"for violation(s)not listed above. <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Com <br /> Routine Annual Inspection and Monitoring System Certification: <br /> Class I violation: 315: <br /> 315. The Bravo float and chains in all UDCs (UDC 1/2 and UDC 3/4) were disabled, the arms of the <br /> shear valves were tied down with a metal wire preventing them from tripping. According to the phone <br /> conversation between the service technician and the owner, the owner had tied down the shear valves <br /> because they kept tripping at night and customers could not pump gas. The technician removed the <br /> metal wire and returned the Bravo float and chains to the properly functioning state. The Bravo float and <br /> chains were tested for functionality and passed. Monitoring equipment shall be maintained to be able to <br /> detect a leak and stop the flow of product if leak is detected. <br /> Class II violations: 104, 106, 201, 204, 205, 305, and 311: <br /> 104. An updated copy of Certification of Financial Responsibility was received by EHD on June 21, <br /> 2013, however the received copy lacked the signature of a witness or a notary. Current financial <br /> responsibility documents are required to be submitted annually. Complete and submit a copy of the <br /> financial responsibility by July 25, 2013. REPEAT VIOLATION. <br /> 106. The monitoring plan uploaded to California Environmental Reporting System (CERS) is not current <br /> and not approved by the EHD. The monitoring and response plans must be current and approved by the <br /> EHD. Log in to the CERS and update the monitoring plan by July 25, 2013. <br /> 201. No recent alarm history was present on-site upon arrival on the day of the inspection. The last <br /> available alarm history was attached to the April 25, 2013 DO inspection report. The last manual daily <br /> inspection log entry was dated January 12, 2012. Written monitoring and maintenance records shall be <br /> maintained on-site for a period of at least 3 years.
