Wilson., Stockton, CA Program: UST <br /> SUMMARY OF VIOLATIONS <br /> CLASS 1,CLASS 11 or MINOR-Notice to Comply) <br /> 204. The designated operator failed to document all the alarms from the attached alarm history on the <br /> July 26, 2012 designated operator monthly inspection report. The missing alarms include: premium <br /> sump sensor#3 on June 28, 2012, annular space sensor#1 on June 29, 2012, regular sump sensor#2 <br /> and annular space sensor#1 on July 10, 2012. Maintenance records for above alarms were located on <br /> site. During the monthly inspection, the designated operator shall review the alarm history for the <br /> previous month, check that each alarm was documented and responded to appropriately, and attach a <br /> copy of the alarm history with documentation taken in response to any alarms to the monthly report. <br /> Ensure that designated operators performing monthly inspections at this facility are including all of the <br /> required information on the reports. Submit copies to the EHD by July 25, 2013. <br /> 204. The designated operator failed to visually inspect the station for leaks, water, or debris during the <br /> May 2013 monthly inspection. During the monthly inspection, the designated operator shall visually <br /> inspect all spill containers and UDC sumps, and any tank top sump that had an alarm for which there is <br /> no record of a service visit. Ensure that designated operators performing monthly inspections at this <br /> facility are inspecting all of the required areas and noting it on the on the reports. <br /> 205. The May 2013 designated operator monthly inspection report was not found on site. Designated <br /> operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and <br /> ensure that copies of the previous twelve months of designated operator monthly inspection reports are <br /> maintained on site. Submit copies to the EHD by July 25, 2013. <br /> 305. The Bravo box float and chains in all the UDC 1/2 and UDC 3/4 sump failed to stop the flow of <br /> product at the dispenser because the arms of the shear valves were tied down with a metal wire <br /> preventing them from tripping. All monitoring equipment shall be maintained to activate an audible and <br /> visual alarm or stop the flow of product at the dispenser when it detects a leak. The service technician <br /> cut and removed all wires and verified functionality. <br /> 311. The 87-octane line leak detector failed to detect a leak when tested. All line leak detectors shall be <br /> capable of detecting a 3-gallon per hour leak at 10 psi. The service technician adjusted the leak <br /> detector, retested it, and it passed. REPEAT VIOLATION. <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by July 25, 2013. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector.
