Environmental Health Department Underground Storage Tank Program Inspection Report Date: May 04, 2018 Facility Address: 2115 W YOSEMITE AVE, MANTECA Facility Name: 7 ELEVEN 2369-39858A SUMMARY OF VIOLATIONS (CLASS I, CLASS II, or MINOR - Notice to Comply) RemarksItem # 111 CCR 2638(a) Failed to test monitoring and leak detection equipment annually. Annual monitoring system certification and leak detector testing were last performed on 03/29/2018 and was due next calender year. Monitoring system certification testing is being performed on 05/04/18, and is two months past due. These tests are required once every 12 months. Take all necessary precautions to ensure testing is performed in a timely manner. This is a repeat violation, Class II. 115 CCR 2715(a) Failed to submit to EHD notification of designated operator identification or change w/in 30 days. A new designated operator was chosen since the last monitoring certification and notification was not provided to the EHD within 30 days of the change. ECO-Check is the designated operator. The current Designated operator form on CERS indicates Afforda-test as the DO Any changes shall be reported to the EHD within 30 days. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. This is a minor violation. 201 CCR 2712(b) Failure to maintain monitoring, maintenance, and follow up action records. The facility, and the designated operator failed to check that alarms were responded to appropriately. The missing alarms include an L23 Dispenser 1-2 brine alarm that occurred on the designated operator monthly inspection reports dated: 2/15/18, 1/18/18, 12/21/17, 11/17/17, and 10/27/17. During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a minor violation. 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to check and note the correct test dates for the last monitoring system certification that occurred on 3/29/17 on the April 2017 through April 2018 designated operator monthly inspection reports. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a minor violation.
