Lodi, CA 95240 Program: UST <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Com <br /> On site to witness the monitoring system certification testing and to perform the routine inspection. <br /> #2 The secondary containment test was conductedl2-11-08, but the test results were not received till <br /> 1-16-09. Test results must be submitted within thirty days of the test to this office within 30 days of the <br /> test. Correct immediately by ensuring that test results are submitted to this office within 30 days. <br /> #23(a) The Designated Operator report of February 2010 was performed by J. Brown but this <br /> department was not notified within 30 days of this change of the designated operator for this facility. <br /> Correct immediately by notifying this office within 30 days of a change of designated operator. <br /> #23(c) Designated Operator(D.O.) reports from January/February 2010 and November 2009 list the <br /> monitoring system certification, spill container, and line leak detector testing as 12-17-08, but should be <br /> listed as 10-30-09. The D.O. report from December 2009 lists the monitoring system certification, spill <br /> container, and line leak detector testing as 10-29-09, but should be listed as 10-30-09. The November <br /> 2009 D.O. report does not have listed the Nov. 23, 2009 fuel alarms from each of the stp sump sensors <br /> and annular sensors. Correct immediately by ensuring that the D.O. monthly reports are completed <br /> correctly and completely including listing of monitoring system alarms, what was done to correct the <br /> alarm, attach any work orders that document what was done to correct the alarm, and documenting the <br /> correct dates of required testing. <br /> Complete and Submit the return to compliance certification, corrective action statements for each <br /> violation, and any supporting documentation to this office by 11-6-10. <br /> Note: The monitoring system certification test results from 2009 have the date listed as 10-29-09. It <br /> should be 10-30-10.
