December 21,2009 <br /> Elite IV <br /> 2538 Wigmam Drive <br /> Stockton, CA 95205 — F-4 <br /> Re: Wine Country Station, 1111 E Kettleman Lane,Lodi 95204 <br /> The permit(SR0058879)for cold start of the Gilbarco EMC monitoring panel for the underground <br /> storage tank system(UST)at the above referenced site has been approved with the following conditions. <br /> Conditions of approval: <br /> 1. Contact the local Fire Department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits. <br /> 2. Any changes shall be submitted to the San Joaquin County Environmental Health Department <br /> (EHD) for approval prior to commencing work. All new addenda supercede the previous ones. <br /> 3. Contact Thuy Tran(468-8257)to arrange the following inspections (a 48-hour notice is required). <br /> a. EHD must witness the functionality of the monitoring system to ensure that monitoring <br /> has been restored to meet the requirements of California Code of Regulations Title 23 <br /> and the Health& Safety Code. <br /> b. The Post-Cold Start Functionality Test. Since a"cold start"will be conducted,before <br /> the alarm history is cleared by the"cold start", a current printout must be created and <br /> available for EHD inspection. <br /> Please be aware that the facility cannot operate before the <br /> EHD inspection and approval. <br /> 4. All components must be certified by an independent testing organization. (UL,ULC,ANSI,API, <br /> ASTM, etc. See California Code of Regulations(CCR),Title 23 Chapter 16 Table B for <br /> complete list). The component must bear the appropriate mark as required by the independent <br /> testing organization. <br />
