The site map is missing the general layout of the <br />underground piping, and leak detector. A site plan must be submitted identifying the locations where monitoring will <br />be performed. Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/ and upload a copy of the UST Monitoring Site Plan. <br />This is a repeat violation, Class II. <br /> 121 CCR 2712(f) Failed to submit a written response within 30 days of receiving an inspection report. <br />An inspection was last done on 05/26/17 and an inspection report was issued identifying information to be submitted <br />to bring this site into compliance. This information was required to be submitted by 06/25/17 This information has <br />not been received resulting in a non-compliant status for this facility. An operator that receives an inspection report <br />shall have 30 days to submit a written response that includes a statement documenting corrective actions taken or <br />proposing corrective actions which will be taken. Ensure that a written response documenting corrective actions <br />taken or proposed is submitted within 30 days of receiving an inspection report. <br />Violation include: <br />Designated operator did not inspect all required items and/or the inspection reports not <br />completed. <br />1) The designated operator failed to document all the alarms from the attached alarm history on the following <br />designated operator monthly inspection reports and failed to check that they were responded to appropriately: <br />6-27-2016 DO report (L2 alarm is listed but no alarm date is identified), 7-20-2016 (L2 alarm on 7-18-2016). <br />2) The designated operator failed to include alarm histories for the following designated operator inspection reports: <br />7-20-2016, 8-24-2016, and 12-20-2016. During the monthly inspection, the designated operator shall review the <br />alarm history for the previous month, check that each alarm was documented and responded to appropriately, and <br />attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure <br />that designated operators performing monthly inspections at this facility are including all of the required information <br />on the reports. <br />This is a Class II violation. <br /> 210 CCR 2715(e) Designated operator monthly inspection reports not maintained on site for at least 12 months. <br />The September 2017 designated operator monthly inspection reports were not found on site. Designated operator <br />monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure that copies <br />of the previous twelve months of designated operator monthly inspection reports are maintained on site.
