An approved copy of the monitoring and response <br />plans were not found on site. A copy of these plans shall be retained on site at all times. Immediately locate and retain <br />a copy on site. This is a repeat violation, Class Il. <br />205 <br />Failed to record or report unauthorized release in appropriate time frame. The leak in dispenser 5/6 was not recorded. <br />Any release from the primary containment that does not escape the secondary containment and was cleaned up within <br />eight hours of being detected shall be recorded with the following information: <br />1. Operator's name and phone number <br />2. List of types, quantities, and concentrations of hazardous substances released <br />3. Description of actions taken to control and clean the release <br />4. Method and location of disposal of the released substances <br />5. Description of actions taken to repair and prevent future releases <br />6. Description of method used to reactivate interstitial monitoring after repair or replacement of primary containment <br />Immediately record this release with the information required, and maintain it on site with the facility's monitoring <br />records. Submit a copy of the recordable release statement to the EHD. This is a minor violation. <br />208 <br />Designated operator employee training not performed or log not kept. The designated operator employee training was <br />not current. Records on site indicated that the most recent designated operator training was performed on March 28, <br />2013. The designated operator shall train facility employees for which he or she is responsible in the proper operation <br />and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the <br />training records to the EHD. This is a repeat violation, Class ll. <br />306 <br />Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. The UDC 3/4 sump <br />sensor was raised approximately 10 inches off the lowest point of the sump and not located to detect a leak at the <br />earliest opportunity. Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible <br />opportunity. The sensor was relocated to the lowest point of the sump by the service technician. Ensure that all <br />monitoring equipment is maintained to detect a leak at the earliest opportunity. This is a Class II violation. <br />314 <br />Primary containment not tight (pre -Jul 2003). The 87 piping under dispenser 5/6 was observed to be weeping and <br />dripping, indicating a leak in the primary piping. A small amount of liquid was observed in the UDC. All primary <br />containment for the UST system must be tight. Immediately have a properly licensed, trained, and certified contractor <br />repair or replace the failed component under permit and inspection of the EHD. This is a Class II violation. <br />319 <br />All releases in secondary piping do not flow to a collection sump. The test boots were not pulled back in the 87 and 91 <br />STP sumps. The schrader valves pointed straight up. Any potential release from the primary containment into the <br />secondary containment in the product piping does not flow into a collection sump. All secondary containment systems <br />shall be sloped so that all releases will flow to a collection sump located at the low point of the underground piping. The <br />technician pulled back the test boots at the time of inspection. This is a Class II violation. <br />321 <br />Leak detector failed to detect a 3.0 gph leak at 10 psi. The 87 line leak detector (FE Petro STP -MLD) failed to detect a <br />leak when tested. All line leak detectors shall be capable of detecting a 3 -gallon per hour leak at 10 psi. The service <br />technician hit the top of the leak detector with a hammer, retested it, and it passed. Take all necessary precautions to <br />ensure the leak detectors are fully functioning at all times.
