UST Tank Information forms for the 87 and 91 tanks are not current in CERS. Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information, and submit for review by the EHD. Complete a separate tank form for each tank. This is a repeat violation, Class II. 106 Monitoring and response plans not current or approved by the EHD. The monitoring and response plans are not current and/or not approved by the EHD. The monitoring plans submitted are incomplete; the portions that are filled in are inaccurate. The response plan submitted is incomplete. The monitoring and response plans must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. Maintain an approved copy on site. This is a repeat violation, Class II. 107 Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately submit an accurate copy of the UST Monitoring Site Plan to the EHD for approval. Alternatively, log into CERS, upload the completed form, and submit for review by the EHD. Maintain an approved copy on site. This is a minor violation. 111 Failed to test monitoring and leak detection equipment annually. Annual monitoring system certification, leak detector testing, and spill container testing was scheduled to be performed on June 18, 2014 and a test report was submitted to the EHD on August 23, 2013, more than 30 days later. A copy of the test report must be submitted within 30 days of these tests. Ensure that all test reports are submitted within the required time frames. This is a repeat violation, Class ll. 122 Permit to repair UST system not obtained. The 87 STP relay was replaced on March 7, 2014 and a permit was not obtained from the EHD. Emergency repairs relating to UST system monitoring and release detection may be performed by a properly licensed, trained, and certified contractor pursuant to the EHD'S policy and a permit obtained within one business day from the EHD. All other repairs must have a permit submitted to and approved by the EHD prior to beginning work. Immediately obtain a permit from the EHD for the relay replacement.
