UST Tank Information <br />forms for the 87 and 91 tanks are not current in CERS. Any change of information must be updated in CERS within 30 <br />days of the changes. Immediately log into CERS, update the required information, and submit for review by the EHD. <br />Complete a separate tank form for each tank. This is a repeat violation, Class II. <br />106 <br />Monitoring and response plans not current or approved by the EHD. The monitoring and response plans are not current <br />and/or not approved by the EHD. The monitoring plans submitted are incomplete; the portions that are filled in are <br />inaccurate. The response plan submitted is incomplete. The monitoring and response plans must be uploaded to the <br />California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and <br />submit for review by the EHD. Maintain an approved copy on site. This is a repeat violation, Class II. <br />107 <br />Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST <br />Monitoring Site Plan was not submitted. A site plan must be submitted identifying the locations where monitoring will be <br />performed. Immediately submit an accurate copy of the UST Monitoring Site Plan to the EHD for approval. <br />Alternatively, log into CERS, upload the completed form, and submit for review by the EHD. Maintain an approved copy <br />on site. This is a minor violation. <br />111 <br />Failed to test monitoring and leak detection equipment annually. Annual monitoring system certification, leak detector <br />testing, and spill container testing was scheduled to be performed on June 18, 2014 and a test report was submitted to <br />the EHD on August 23, 2013, more than 30 days later. A copy of the test report must be submitted within 30 days of <br />these tests. Ensure that all test reports are submitted within the required time frames. This is a repeat violation, Class <br />ll. <br />122 <br />Permit to repair UST system not obtained. The 87 STP relay was replaced on March 7, 2014 and a permit was not <br />obtained from the EHD. Emergency repairs relating to UST system monitoring and release detection may be performed <br />by a properly licensed, trained, and certified contractor pursuant to the EHD'S policy and a permit obtained within one <br />business day from the EHD. All other repairs must have a permit submitted to and approved by the EHD prior to <br />beginning work. Immediately obtain a permit from the EHD for the relay replacement. A "Failure to Obtain a Permit" <br />penalty applies.
