Public Health Services <br /> July 13, 1998 <br /> Page 2 <br /> The report for 1331 S. Ham Lane, listed seven items. The following responses are for <br /> Items 1, 2, 3, 4, and 7: <br /> Municipal Service Center (MSCI <br /> • Item 1 - Enclosed is report from Shields & Harper stating they have serviced <br /> Tank No. 4's alarm system and it is working. <br /> • Item 2 - Enclosed are the results showing that all the monitoring equipment at the <br /> MSC has been checked and is in running condition. <br /> • Item 3 - Enclosed are the results for the annual product line tests showing passing <br /> condition. <br /> • Item 4 - Enclosed is the cathotic protection report showing that two of the four tanks <br /> at this location were not receiving adequate CP.
