SUMMARY <br /> On October 3,2002,the above referenced facility was mailed a San Joaquin County <br /> Environmental Health Department(EHD) letter(Attachment 1)reminding underground <br /> storage tank(UST)owners and operators that the secondary containment testing must be <br /> performed by January 1, 2003. A second EHD letter regarding this requirement was sent <br /> via certified mail on November 15,2002(Attachment 2). <br /> On January 17,2003,the underground storage tank(UST) facility was considered out of <br /> compliance(Attachment 3)because no secondary containment test results had been <br /> submitted to the EHD. <br /> March 27,2003, a routine UST inspection was performed at the above referenced facility by <br /> Mr. Steven Shih. It was noted in the inspection report(Attachment 4)that the secondary <br /> containment results had not been received by the EHD. The facility was informed that its <br /> permit would not be renewed due to its non-compliance status. Secondary containment <br /> testing was performed on this date for the two gasoline tank systems(including piping and <br /> sumps), but was not done on the diesel and waste oil tanks piping and sumps due to missing <br /> test boots. The financial responsibility and letter from the Chief Financial Officer had also <br /> not been submitted for 2003. <br /> On April 9, 2003,the secondary containment test results dated March 27, 2003,were <br /> received by the EHD. Results showed that sumps and piping for run#3 (diesel)and run 94 <br /> (waste oil)were not tested(Attachment 5). <br /> On April 29, 2003,the certification of financial responsibility and letter from the Chief <br /> Financial Officer was received by the EHD. <br /> On May 28, 2003,the EHD received a fax (Attachment 6) describing the repairs to be made <br />
