Ham Ln., Lodi <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of <br /> the application for a permit to replace the drain valve and replace the overfill alarm on the <br /> 10,000 gallon unleaded tank on the underground storage tank (UST) system at the above noted <br /> address. The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Aris Cacapit at (209) 468-3453 to arrange the following inspection appointments <br /> (a 48-hour notice is required for inspection appointments). Please be aware that <br /> inspections for emergency work must be scheduled within five days of permit approval. <br /> a. Overfill prevention verification: Verify that external audio and visual alarm activates <br /> when tank is 90% full. <br /> b. Spill bucket test: Verify minimum 5-gallon capacity. Proper drainage must also be <br /> verified. <br /> Please be aware that the facility cannot receive fuel in the 10,000 gallon unleaded <br /> tank prior to the EHD inspection and approval. <br /> 4. If concrete will be broken near the UST, a secondary containment test must be <br /> performed on the UST once concrete is poured in accordance with the manufacturer's <br /> guidelines. <br /> 5. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br />
