The following is an itemized list of hazardous waste violations that have not been addressed for Quick Stop Mart as of June 10, 2015. Open violations from April 21, 2015 inspection Violation #101 - Managed a hazardous waste without a valid EPA ID number. This facility's EPA ID number is inactive. A hazardous waste generator shall not treat, store, dispose of, transport or offer for transportation, hazardous waste without an EPA ID number. Immediately contact DTSC and reactivate your EPA ID number and submit evidence to the EHD. A form has been provided. Violation #109 - Failed to keep signed copy of manifests from the designated facility for three years. Copies of uniform manifests for the disposal of hazardous waste for the last three years were not found on site. Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts used in a consolidated manifesting procedure on site for three years and have them readily available for review. Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit copies to the EHD. Violation #116 - Failed to submit a written response within 30 days of receiving an inspection report. An inspection was last done on April 7, 2014 and an inspection report was issued identifying information to be submitted to bring this site into compliance. This information was required to be submitted by May 7, 2014. This information has not been received resulting in a non-compliant status for this facility. A deficiency letter was issued on June 2, 2014. Return to Compliance paperwork for the April 1, 2013 and December 16, 2011 inspections was also not submitted. A deficiency letter for the 2011 inspection was issued July 23, 2012. The outstanding violations from 2011 include: -CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for 3 years -HSC 25160.2(b)(3) Failed to keep a copy of the consolidated manifest for 3 years -CCR 66266.130(c)(5) Failed to retain a copy of bill of lading for used oil filters for 3 years -CCR 66262.34(f) Failed to completely label containers or tanks of hazardous waste -CCR 66262.42(c) Failed to send copy of generator manifest to DTSC with an indication that copy of manifest signed by destination facility was not received after 60 days -CCR 66273.39 Failed to keep records of each shipment of universal waste according to this section The outstanding violations from 2013 include: -CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for 3 years -HSC 25185(c)(3) Failed to submit a written response within 30 days of receiving an inspection report -CCR 66262.34(f) Failed to completely label containers or tanks of hazardous waste -CCR 66262.34(d) Stored hazardous waste onsite greater than 180 days -CCR 66262.42(c) Failed to send copy of generator manifest to DTSC with an indication that copy of manifest signed by destination facility was not received after 60 days The outstanding violations for 2014 include: -CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for 3 years -HSC 25185(c)(3) Failed to submit a written response within 30 days of receiving an inspection report -CCR 66261.7(f)An empty HW or Haz Mat container(larger than 5 gallons)was not marked with the date it was emptied and/or not managed within one year of being emptied -CCR 66262.34(d) Failed to comply with Code of Federal Regulations 262.34(d), (e), (f). No emergency coordinator. No modified contingency plan. Failed to train all employees on waste handling and emergency procedures. Failed to respond to a fire, spill, or other release. -CCR 66262.42(c) Failed to send copy of generator manifest to DTSC with an indication that copy of manifest signed by destination facility was not received after 60 days An operator that receives an inspection report shall have 30 days to submit a written response that includes a statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. Page 1 of 5
