The following is an itemized list of hazardous waste violations that have not been addressed <br /> for Quick Stop Mart as of June 10, 2015. <br /> Open violations from April 21, 2015 inspection <br /> Violation #101 - Managed a hazardous waste without a valid EPA ID number. <br /> This facility's EPA ID number is inactive. A hazardous waste generator shall not treat, store, dispose of, transport <br /> or offer for transportation, hazardous waste without an EPA ID number. Immediately contact DTSC and <br /> reactivate your EPA ID number and submit evidence to the EHD. A form has been provided. <br /> Violation #109 - Failed to keep signed copy of manifests from the designated facility for three years. <br /> Copies of uniform manifests for the disposal of hazardous waste for the last three years were not found on site. <br /> Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts <br /> used in a consolidated manifesting procedure on site for three years and have them readily available for review. <br /> Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit <br /> copies to the EHD. <br /> Violation #116 - Failed to submit a written response within 30 days of receiving an inspection report. <br /> An inspection was last done on April 7, 2014 and an inspection report was issued identifying information to be <br /> submitted to bring this site into compliance. This information was required to be submitted by May 7, 2014. This <br /> information has not been received resulting in a non-compliant status for this facility. A deficiency letter was <br /> issued on June 2, 2014. Return to Compliance paperwork for the April 1, 2013 and December 16, 2011 <br /> inspections was also not submitted. A deficiency letter for the 2011 inspection was issued July 23, 2012. The <br /> outstanding violations from 2011 include: <br /> -CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for 3 years <br /> -HSC 25160.2(b)(3) Failed to keep a copy of the consolidated manifest for 3 years <br /> -CCR 66266.130(c)(5) Failed to retain a copy of bill of lading for used oil filters for 3 years <br /> -CCR 66262.34(f) Failed to completely label containers or tanks of hazardous waste <br /> -CCR 66262.42(c) Failed to send copy of generator manifest to DTSC with an indication that copy of manifest <br /> signed by destination facility was not received after 60 days <br /> -CCR 66273.39 Failed to keep records of each shipment of universal waste according to this section <br /> The outstanding violations from 2013 include: <br /> -CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for 3 years <br /> -HSC 25185(c)(3) Failed to submit a written response within 30 days of receiving an inspection report <br /> -CCR 66262.34(f) Failed to completely label containers or tanks of hazardous waste <br /> -CCR 66262.34(d) Stored hazardous waste onsite greater than 180 days <br /> -CCR 66262.42(c) Failed to send copy of generator manifest to DTSC with an indication that copy of manifest <br /> signed by destination facility was not received after 60 days <br /> The outstanding violations for 2014 include: <br /> -CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for 3 years <br /> -HSC 25185(c)(3) Failed to submit a written response within 30 days of receiving an inspection report <br /> -CCR 66261.7(f)An empty HW or Haz Mat container(larger than 5 gallons)was not marked with the date it was <br /> emptied and/or not managed within one year of being emptied <br /> -CCR 66262.34(d) Failed to comply with Code of Federal Regulations 262.34(d), (e), (f). No emergency <br /> coordinator. No modified contingency plan. Failed to train all employees on waste handling and emergency <br /> procedures. Failed to respond to a fire, spill, or other release. <br /> -CCR 66262.42(c) Failed to send copy of generator manifest to DTSC with an indication that copy of manifest <br /> signed by destination facility was not received after 60 days <br /> An operator that receives an inspection report shall have 30 days to submit a written response that includes a <br /> statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that <br /> a written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an <br /> inspection report. <br /> Page 1 of 5 <br />
