The following is an itemized list of hazardous waste violations that have not been addressed <br /> for Quick Stop Mart as of June 10, 2015. <br /> Open violations from April 21, 2015 inspection <br /> Violation #303 -Failed to maintain all communication or alarm systems, spill control, or decontamination <br /> equipment. <br /> The fire extinguisher for this facility was observed with an annual maintenance date of May 2015, but with a <br /> gauge indicating it needs to be recharged. All facility communications or alarm systems, fire protection <br /> equipment, spill control equipment, and decontamination equipment, where required, shall be tested and <br /> maintained as necessary to assure its proper operation in time of emergency. Immediately conduct all necessary <br /> testing and maintenance on the fire extinguisher(s) at this facility and begin this practice for all emergency <br /> equipment. <br /> Violation#601 -Stored hazardous waste onsite greater than 180 days. <br /> A 55 gallon blue steel drum of hazardous waste liquid was observed on site. According to the facility contacts, <br /> this facility has not generated waste liquids in over six months. Facilities who generate less than 1000 kg of <br /> hazardous waste per month and do not exceed 6000 kg of waste stored on site at any time may store waste on <br /> site up to 180 days. Immediately contact a licensed hazardous waste hauler to dispose of this waste under <br /> manifest and submit a copy of the manifest to the EHD. <br /> Violation #605- Failed to completely label containers or tanks of hazardous waste. <br /> A 55 gallon blue steel drum of hazardous waste liquid was observed in the hazardous waste storage area with a <br /> blank hazardous waste label. All hazardous waste containers shall be marked with the following information: <br /> -the words"Hazardous Waste" <br /> - name and address of generator <br /> - hazardous properties <br /> - physical state (liquid, solid) <br /> -composition (contents) <br /> -accumulation start date <br /> Immediately label this container and ensure that all containers are marked with all the required information. Since <br /> this containers has been on site longer than 180 days, immediately contact a licensed hazardous waste hauler to <br /> dispose of this waste under manifest and submit a copy of the manifest to the EHD. <br /> Violation #608 -Failed to properly manage used oil and fuel filters. <br /> Bills of lading for the recycling of used oil filters for the last three years were not found on site. A copy of each bill <br /> of lading must be kept on site for at least three years. Immediately locate a copy of all bills of lading for used oil <br /> filters for the last three years, maintain them on site, and submit copies to the EHD. <br /> Open violations from April 07, 2014 inspection <br /> Violation #101 - Managed a hazardous waste without an EPA number(unless exempted HSC 25143.13) <br /> This facility's EPA ID number is inactive. A hazardous waste generator shall not treat, store, dispose of, transport <br /> or offer for transportation, hazardous waste without an EPA ID number. Immediately contact DTSC and <br /> reactivate your EPA ID number and submit evidence to the EHD by May 7, 2014. A form has been provided. <br /> Page 2 of 5 <br />
