The following is an itemized list of hazardous waste violations that have not been addressed for Quick Stop Mart as of June 10, 2015. Open violations from April 1, 2013 inspection 122 - Failed to keep signed copy of manifests from the designated facility for 3 years 705 - Failed to send copy of generator manifest to DTSC with an indication that copy of manifest signed by destination facility was not received after 60 days Manifest number 007271639JJK(7/26/11)was not found on site. Hazardous waste generators shall retain copies of all manifests for three years or until a copy signed off by the destination facility is received. The signed off copy shall be retained and made readily available for review for three years. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 60 days of the date the waste was accepted by the initial transporter, the generator shall submit a legible copy of the missing manifest, with some indication that the generator has not received confirmation of delivery. This information shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifest or prepare and submit the required information to DTSC. Immediately submit to the EHD a copy of the signed off copy of the manifest or a copy of the information submitted to DTSC. REPEAT VIOLATION 135 - Failed to submit a written response within 30 days of receiving an inspection report The complete Return to Compliance paperwork for the December 21, 2011 inspection was not submitted. A deficiency letter was issued on July 23, 2012. The operator of the site or facility which receives an inspection report shall submit a written response to the department or the local officer or agency within 60 days of receipt of the inspection report, or with a shorter time as the department or local agency requires. This response shall include a statement documenting the corrective actions that have been or will be taken by the operator. Immediately submit all necessary follow up paperwork for the 2011 hazardous waste generator inspection to the EHD. 605 - Failed to completely label containers or tanks of hazardous waste A 55 gallon blue steel drum of waste liquids was observed in the hazardous waste storage area with faded, illegible hazardous waste labels. A 55 gallon black steel drum of waste liquids was observed in the hazardous waste storage area without labeling. All hazardous waste containers shall be marked with the following information: 1. the words"Hazardous Waste" 4. physical state (liquid, solid) 2. name and address of generator 5. composition (contents) 3. hazardous properties (flammable, corrosive, reactive, toxic) 6. accumulation start date Immediately label these containers and ensure that all hazardous waste containers are marked with all the required information. Since it cannot be determined how long the black drum has been on site, immediately contact a licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of the manifest to the EHD by May 1, 2013. REPEAT VIOLATION 701 - Stored hazardous waste onsite greater than 180 days The 55 gallon blue steel drum of waste liquids on site was observed during the December 21, 2011 inspection. Facilities who generate less than 1000 kg of hazardous waste per month and do not exceed 6000 kg of waste stored on site at any time may store waste on site up to 180 days. Immediately contact a licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of the manifest to the EHD by May 1, 2013 Page 4 of 5
