The following is an itemized list of hazardous waste violations that have not been addressed <br /> for Quick Stop Mart as of June 10, 2015. <br /> Open violations from December 16, 2011 inspection <br /> Violation #122 - Failed to keep signed copy of manifests from the designated facility for 3 years <br /> Violation #705 - Failed to send copy of generator manifest to DTSC with an indication that copy of <br /> manifest signed by destination facility was not received after 60 days <br /> Manifest number 007271639JJK(7/26/11)was not found on site. Hazardous waste generators shall retain copies <br /> of all manifests for three years or until a copy signed off by the destination facility is received. The signed off copy <br /> shall be retained and made readily available for review for three years. If the generator did not receive a copy of <br /> the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste <br /> was submitted within 60 days of the date the waste was accepted by the initial transporter, the generator shall <br /> submit a legible copy of the missing manifest, with some indication that the generator has not received <br /> confirmation of delivery. This information shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifes or prepare and submit the required information to DTSC. By <br /> January 16, 2012, submit to the EHD a copy of the signed off copy of the manifest or a copy of the information <br /> submitted to DTSC. REPEAT VIOLATION <br /> Violation #605 - Failed to completely label containers or tanks of hazardous waste <br /> A 55 gallon blue steel drum of waste liquids, a 55 gallon black steel drum of used oil filters and other waste solids, <br /> and a 5 gallon orange bucket of waste absorbent were observed in the hazardous waste storage area with faded <br /> hazardous waste labels that could not be read. A 55 gallon black steel drum of waste liquid and a 5 gallon white <br /> bucket of waste paint were observed in the hazardous waste storage area without labeling. All hazardous waste <br /> containers shall be marked with the following information: <br /> 1. the words "Hazardous Waste" <br /> 2. name and address of generator <br /> 3. hazardous properties (flammable, corrosive, reactive, toxic) <br /> 4. physical state (liquid, solid) <br /> 5. composition (contents) <br /> 6. accumulation start date <br /> Immediately label these containers and ensure that all containers are marked with all the required information. <br /> Since it cannot be determined how long the unlabeled wastes have been on site, immediately contact a licensed <br /> hazardous waste hauler to dispose of this waste under manifest and submit a copy of the manifest to the EHD by <br /> January 16, 2012. REPEAT VIOLATION <br /> Violation#812 - Failed to keep records of each shipment of universal waste according to this section <br /> Copies of universal waste disposal records were not found on site. A generator of universal waste (alkaline <br /> batteries, mercury containing equipment, fluorescent bulbs, electronic devices, CRTs, aerosol cans) shall keep a <br /> record of each shipment of universal waste on site for at least three years. The record may take the form of a log, <br /> invoice, manifest, bill of lading, or other shipping document, and shall include the following: <br /> - Name & address of destination facility <br /> - Quantity &type of universal waste <br /> - Date of departure/shipment <br /> Begin this practice immediately. <br /> Page 5 of 5 <br />
