MONITORING AND REPORT•PROGRAM NO. 5-01-XXX • UNOCAL CORPORATION,AND AGRIUM,INC. -3 - STOCKTON, SAN JOAQUIN COUNTY Any necessary repairs or routine maintenance to the cover, cover slope, and drainage control system shall be completed by 15 October of each year, unless an extension is granted by the Executive Officer. Routine maintenance should consist of, at a minimum, annual crack sealing and seal coating every five years (or more frequently if visual inspection dictates the need). Results of the annual inspection and maintenance activities shall be included in the Annual Report. PERMANENT SURVEY MONUMENT MONITORING Every 10 years, the Discharger shall survey the location of the permanent survey monuments. The results of the first re-survey shall be included in the 30 October 2006 report. The surveys shall be compared to previous surveys and the results submitted to the Board. REPORTING When reporting data, the Discharger shall arrange the information in tabular form so that the date, the constituents, and the concentrations are readily discernible. The data shall be summarized in such a manner as to illustrate clearly the compliance with this Order. In addition, the Discharger shall notify the Board within 48 hours of any unscheduled shutdown of any soil vapor and/or groundwater extraction system. As required by the California Business and Professions Code Sections 6735, 7835, and 7835.1, all reports shall be prepared by a registered professional or their subordinate and signed by the registered professional. Semi-annual reports shall be submitted to the Board by the 30th day of the month following the end of the respective quarter (i.e., by 30 April and 30 October) until such time as the Executive Officer determines that the reports are no longer necessary. Each semi-annual report is to include the following minimum information: (a) a description of the semi-annual groundwater sampling event, including field logs. At a minimum, field logs shall contain water quality parameters measured before, during, and after purging,method of purging, depth of water, volume of water purged, etc.; (b) groundwater contour maps for all groundwater zones, if applicable; c isocontour contaminant concentration maps for all groundwater zones, if applicable; (d) groundwater hydrograph(s) charting groundwater elevations with time for all monitoring wells; (e) graph(s)portraying constituent concentrations with time for all monitoring wells;
