Can you <br />please provide the agency and a contact for who issued the fire department <br />permit for this site? <br />2) Check all the appropriate boxes through the application, mine came <br />through unchecked. All appropriate boxes have been checked. <br />3) Different overfill model is identified p 7 of 8 as compared to the <br />original application. I do not know what was submitted on the original <br />application but the part number I provided is 6571 EVRB- I 64042RT052 is for <br />the entire spill kit. Are you looking for the entire spill kit or just the <br />bucket? Just the bucket (which is part of the spill kit) part number is <br />P761 C-FLDV. I changed the overfill prevention device from 7150-410C to <br />71 S0420C as this was incorrect. <br />4) P 8 of 8 only lists one sump model, two sumps 42" and 46" are being <br />proposed. I added the second sump model to that application on page 8 to <br />show 2 sumps. <br />5) P 8 of 8 only one UDC size is listed two were proposed before. I <br />added the second UDC model to the application on page 8 to show 2 UDC's. <br />6) On service request please check the correct billing party and please <br />confirm that Chadha Construction will be the owner/operator of these USTs, <br />if not please include the correct owner/operator. I have checked the box <br />stating Chadra Construction is the owner/opertator of the UST's. <br />7) Please provide Fire Flex certification for Les Henderson. The Fire <br />Flex certification for Les Henderson is included in the provided Franklin <br />Fueling Containmet (APT Brand) Certification. The fire <br />8) Need ICC certification for Marcus Garcia. I have attached ICC <br />Certification for Marcus Garcia. <br />
