October 5, 2007 <br /> San Joaquin County <br /> Environmental Health Department <br /> 209 468-9966 <br /> Jenny Frase <br /> Robert McClellan <br /> Jenny and Robert: <br /> In response to your visit on 10/02/2007, here is a written report of the matters we <br /> discussed pertaining to the inspection report#I6-1010738 at 3550 N. Wilson Way dated <br /> 05/15/2007. I am including folders labeled with trailer#'s with the corresponding <br /> documents in them. <br /> I will begin with the activity leading up to the 7 containers (trailers) in question <br /> containing tires at this location. <br /> Trailer#1 —54' trailer#53070—500 tires: <br /> This container arrived empty to 3550 N. Wilson Way on or around 06/01/2007. <br /> The container was then moved and manifested to adjacent property 3730 N. <br /> Wilson Way on 06/05/2007 on document#3497992. The container was then <br /> picked up by Tri-C tire recycling on 06/21/2007 manifest#3550554. Tri-C driver <br /> listed the incorrect address on their paperwork, and that error was not noted by <br /> our employee. <br /> Trailer#2—27' white trailer, no markings—250 tires: <br /> This container with 389 tires arrived at 3730 N. Wilson way on 05/14/2007 on <br /> manifest#3497801. It was then moved and manifested to adjacent property 3550 <br /> N. Wilson Way on document#3397988 on the same day. After the 05/15/2007 <br /> inspection, (on 06/12/2007)the trailer was moved and manifested back to <br /> adjacent property 3730 N. Wilson Way with a count of 250 tires on manifest <br /> #3497939. This count was used as a result of the inspection report#I6-1010738 <br /> dated 05/15/2007. Jaime Alvarez, in order to reference and account for this <br /> container, used the quantity from the inspection report so as to avoid confusion. <br /> This matter has been dealt with internally, and Jaime now realizes that the <br /> manifest is an official and legal document, and the numbers must be true and <br /> correct to the best of his knowledge. This act was in no way to be misleading or <br />
