Yosemite Avenue, Manteca <br /> Service Request #0025027 <br /> The proposed plan and application for permit to retrofit the 10,000 gallon underground <br /> storage tank (TA0146405) at the above referenced location, has been approved with <br /> the following conditions: <br /> 1. The retrofit must be completed according to the drawings submitted to the tank <br /> manufacturer. <br /> 2. The retrofit may not include grindings or chemical attachment to the original tank. <br /> 3. The brine reservoir must be 24 hour lake tested under PHS-EHD inspection. <br /> 4. The tank must be appropriately backfilled and covered according to original <br /> installation plan. <br /> 5. Hard copy passing tank precision test results must be submitted before final <br /> permit to operate will be granted. <br /> 6. All equipment installed must be state approved. <br /> 7. The new drop tube installed in the diesel tank must provide positive shut off to <br /> the tank at 95% to satisfy the overfill prevention requirement in the California <br /> Code of Regulations Title 23 Section 2635(b)(2). <br /> 8. All inspections must be scheduled at least 48 hours in advance. <br /> 9. Please contact the local Fire Department and building department for permitting <br /> requirements. <br /> 10.Any changes in the information provided must be submitted to EHD for approval <br /> prior to commencing work. <br /> 11.This permit expires on June 1, 2001. <br /> Please contact me at (209) 468-9848 to schedule inspections and with any questions. <br /> A Division of San Joaquin County Health Care Services <br />
