The following is an itemized list of underground storage tank violations that have not <br /> been addressed for TRACY76 as of August 09, 2017. <br /> Open violations from May 03, 2016 inspection <br /> Violation#209 - Designated operator did not inspect all required items and/or the inspection reports not <br /> completed. <br /> The designated operator failed to check and note the correct test dates for the annual monitoring system <br /> certification and spill container testing on the April 6, 2016 designated operator monthly inspection report. The <br /> designated operator listed April 5, 2016 as the test dates for the monitoring system certification and spill container. <br /> The monitoring system certification and spill container testing with an inspector present were completed on May 3, <br /> 2016. During the monthly inspection, the designated operator shall check that all required testing and maintenance <br /> for the UST system have been completed and document the dates they were done. Ensure that designated <br /> operators performing monthly inspections at this facility are including all of the required information on the reports. <br /> Violation #314- Primary containment not tight (pre-Jul 2003). <br /> Oily debris was found below the shear valve in under dispenser containments for dispensers 1-2, 5-6, and 7-8, <br /> indicating a possible leak in the primary piping. All primary containment for the UST system must be tight. <br /> Immediately have a properly licensed, trained, and certified contractor repair or replace the failed component under <br /> permit and inspection of the EHD. <br /> Open violations from May 18, 2015 inspection <br /> Violation#201 - Failed to maintain alarm logs and/or records of follow up actions. <br /> Maintenance and monitoring records for the last three years were not found on site, specifically, the secondary <br /> containment testing report for testing conducted on May 14, 2012 was not available for review. These records shall <br /> be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or <br /> sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) <br /> results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring <br /> equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. <br /> Immediately locate and maintain all maintenance and monitoring records for the last three years and maintain <br /> records onsite, submit proof of correction to the EHD. <br /> Page 3 of 3 <br />
