The 2016 monitoring system <br /> certification test results were not found on site. These records shall be maintained on site for at least three years. <br /> Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and <br /> maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the <br /> field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) <br /> results of inventory readings and reconciliations. Immediately locate and maintain all missing maintenance and <br /> monitoring records for the last three years on site. <br /> This is a repeat violation, Class II. <br /> 204 CCR 2712(1) Current monitoring plan approved by the EHD not found on site. <br /> An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible <br /> on site at all times. Immediately locate and retain a copy on site. <br /> This is a repeat violation, Class Il. <br /> 309 CCR 2636(f)(1) Monitoring in sump / UDC failed to activate an audible and visual alarm or stop the flow product. <br /> The Bravo box floats and chains in UDC 1/2 (west), 5/6 (west), and 7/8 (east) failed to stop the flow of product at the <br /> dispenser when tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop <br /> the flow of product at the dispenser when it detects a leak. The service technician adjusted the floats and chains and <br /> verified functionality. <br /> This was corrected on site. <br /> This is a repeat violation, Class ll. <br /> 313 HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product-tight. <br /> Dripping fuel was observed in dispenser 7/8, liquid was observed in a light coating of the UDC beneath, and a strong <br /> fuel smell was noted at the UDC, indicating a leak in the primary piping. The rapid drip was only observed while the <br /> pump was running. All primary containment for the UST system must be tight. Immediately have a properly licensed, <br /> trained, and certified contractor repair or replace the failed component under permit and inspection of the EHD. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by June 21, 2018. <br /> Note: The liabilities and assets amounts on the Letter from the Chief Financial Officer have not not changed since <br /> 2014.
