All containers with hazardous waste must have a label with the following information: <br /> The words"Hazardous Waste" <br /> Name and address of the facility <br /> Missing composition (contents) <br /> Accumulation start date <br /> Physical state <br /> Hazardous property <br /> Immediately,properly label all containers storing hazardous waste. <br /> Violation #21-Failed to keep containers of HW closed except when adding/removing <br /> hazardous waste 66265.173(a) CCR. <br /> At time of inspection clear container near cardboard baler labeled "hazardous"was seen open. <br /> Hazardous waste containers must be kept closed at all times unless actively adding or removing <br /> hazardous waste. <br /> Immediately, close all storage containers of hazardous waste unless actively adding <br /> or removing hazardous waste. <br /> Violation #27-Failed to keep signed copy for 3 years 66262.40(a) CCR. <br /> At time of inspection, uniform hazardous waste manifests could not be provided for the disposal <br /> of photo waste. Hazardous waste generators must maintain signed copies of manifests for 3 <br /> years and have them readily available for review. <br /> Within 30 days, March 8, 2009,provide this office with copies of all signed manifests <br /> for the previous 3 years. <br /> Violation # 30-Failed to keep a copy of the consolidated manifest for 3 years 25160.2 <br /> HSC. <br /> At time of inspection copies of consolidated manifest for the previous 3 years could not be <br /> provided. Hazardous waste generators must maintain signed copies of consolidated manifests for <br /> 3 years and have them readily available for review. <br /> Within 30 days, March 8, 2009, submit copies of all consolidated hazardous waste <br /> manifests for the previous 3 years to this office. <br /> Violation #93-Proper waste handling and emergency procedures 40 CFR Section <br /> 262.34(d)(5)(iii) The generator must ensure that all employees are thoroughly <br /> familiar with proper waste handling and emergency procedures, relevant to their <br /> responsibilities during normal facility operations and emergencies. <br /> At time of inspection, several team members were asked how to respond to a hazardous waste <br /> spill. All team members provided different responses. Mike Dias, ETL-Hardlines, stated a <br /> "certified team member"would respond to clean up the spill. Absorbent would be used if <br /> necessary, than the"certified team member"would sweep up the spill, place it in a plastic bag <br /> and put in the back for"receiving"to make a determination of what to do with it. Patrick in <br /> photo stated "he didn't know, he trained himself". Upon asking Cindy in receiving about spill <br /> cleanup policies she stated, "it depends on whose working". Based on the employee's <br /> inconsistent answers, it was not demonstrated to the inspectors that hazardous waste <br /> management procedures had been provided to employees at this facility. <br /> Within 30 days, March 8, 2009,provide proof that employees are thoroughly familiar <br /> with proper waste handling and emergency procedures <br /> Violation #57-Failed to manage incompatible wastes in containers properly 66265.17 <br /> CCR. <br /> At time of inspection, facility was storing mixed waste aerosol cans, which included Hot Shot bug <br /> spray, Target brand disinfectant, baking soda, and fingernail p lis wi u any knowledge of <br /> 4 <br /> EHD Inspect r Received by <br />
