S A N 4J O A Q U l N Environmental Health Department <br /> COUNTY— <br /> Complaint Record Daily Time and Activity Summary from 7/20/2018 to <br /> 12/3/2018 for COEJ1 WQND located at <br /> ARCO 06080, 85 E LOUISE AVE LATHROP, CA95330 <br /> Activity Date Recorded by <br /> 10/01/2018 Called and spoke to Karine (209-323-0880), Store Manager of Arco facility. Karine PE 2546 FLORIDO <br /> 42 min did not know who would be conducting the clean-up, nor when the clean-up would SC 544 <br /> begin, but that the facility was in the process in obtaining a clean-up contractor. <br /> Karine stated the Jamil Kabariti, Franchisee, (510-552-4304)was en route to the <br /> facility now. <br /> Informed Haza Saeed of the incident, since he has the C-pager today. <br /> Generated complaint number and provided complaint information to Haza. <br /> 10/01/2018 7:36 AM- Received a page. Called SJC General Hospital and received a PE 2546 FLORIDO <br /> 18 min complaint regarding a 5-7 gallon gasoline spill at the Arco at 85 E. Louise in SC 907 <br /> Lathrop. Called and spoke to Scott Nausbaumer from the fire department <br /> (209-275-7420)who verified approximately 5-7 gallons of gasoline spilled due to a <br /> broken hose at pump 5. The released is stopped. An unknown amount entered a <br /> nearby city storm drain. Absorbent has been placed on the spillage and the drain <br /> has been diked off. Clean-up arrangements are not yet known, but the facility is <br /> working on getting a clean-up company out there to conduct the clean-up. Scott <br /> stated he was going to call City of Lathrop Public Works to inform them of the spill <br /> to the storm drain. END. <br /> Page 1 of 3 <br />
