SAN _10 A Q U I N Environmental Health Department <br /> COUNTY— <br /> Complaint Record Daily Time and Activity Summary from 7/20/2018 to <br /> 12/3/2018 for COEJ1WQND located at <br /> ARCO 06080, 85 E LOUISE AVE LATH ROP, CA95330 <br /> Activity Date Recorded by <br /> 10/01/2018 At 8:30am Eliana Florido with EHD discussed this complaint with me. AT 8:42am , PE 2546 SAEED <br /> 270 min EHD was notified of the same spill, and confirmed it was the same complaint. At SC 544 <br /> 8:48 1 called the store and spoke with Jamil Kabariti. Jamil told me the City of <br /> Lathrop Public Work was the site, and confirmed that the spill is contained at <br /> property drain and did not leave the site. He told me that NRC Environmental was <br /> called to conduct the cleanup, and he does not have an ETA for them. I asked of <br /> their phone number and told me to call him back in ten minutes. At 9:00am I <br /> called Jamil and I was given NRC phone number(1800-899-4672). 1 called the <br /> number and I was told that the supervisor for this clean-up will call me. NRC <br /> supervisor, Josh Jacki at 510-239-1553 called me, and told me their ETA <br /> 10:30am. I confirmed with him that they are brining equipment to clean-up the <br /> storm drain. <br /> At 09:24am I left the office to respond to this incident. I arrived onsite at 10:43am, <br /> and met with Josh with NRC Environmental. Karen Wollfe, manager onsite <br /> confirmed that public work were onsite, and verified the spill did not leave the site, <br /> but did not have their phone number or names. I called City of Lathrop public <br /> work, (209)992-0028, and spoke with Ian Ziehr and confirmed that the only storm <br /> drains impacted are onsite, and did not leave the site. <br /> Approximately 6 gallons of Gasoline spilled out of dispenser#6, and impacted the <br /> the concrete, asphalt and onsite storm drains. City Of Lathrop Public Work verified <br /> that the the spill did not leave the site. NRC Environmental onsite to conduct the <br /> cleanup. NRC pressured washed the storm drain, and cleaned up the concrete <br /> and asphalt with absorbent, and pressure washed at the end. The UST <br /> technician,Alex Tate was onsite to run a pressure line test, and verified no leak. <br /> generated the Notice to abate and emailed it to the compliance contact, Daryl Lee. <br /> At 12:00pm NRC finished cleaning up the storm drains, and told them they need to <br /> do additional cleanup for the concrete/asphalt. They asked if they should add <br /> more absorbent or pressure wash, and told them whatever method they want to <br /> use as long as they did not contaminate the storm drain again. At 12:45pm, the <br /> cleanup was completed. Three drums of Hazardous waste were generated. I <br /> asked the contractor to label the drums. Drums were labeled, and took photos of <br /> the drums, labels, and final cleanup. The drums will be stored onsite for future <br /> disposal. I left the site at 1 pm. <br /> 10/02/2018 1 completed the Emergency Response Record, Prop 65, and Photo Log, and PE 2546 SAEED <br /> 102 min printed to attach with the report. I added an entry with PR0513584 to follow up with SC 544 <br /> disposal records during the HW routine inspection. I printed the Notice to abate <br /> and attached to the report. I abated the compliant and submitted for review. <br /> 10/25/2018 Kim Blackwell gave me mail with returned Prop 65. 1 verified the mailing address PE 2546 SAEED <br /> 18 min for the property owner in CERS. The mailing address was correct in CERS. Parcel SC 007 <br /> Viewer has a different address. I called the Environmental Contact, Daryl Lee and <br /> asked if the property info. in CERS is correct and told me no. I asked him to <br /> update. I discussed with Garrett Backus the returned mail . Garrett told me not to <br /> spend much time on it and I can email it to Daryl if he does not receive a copy. I <br /> scanned the prop 65 and mail to Daryl, and asked him to forward a copy to the <br /> property owner if they have not received a copy. I attached the returned mail to the <br /> report. <br /> 10/30/2018 Received the disposal records from Daryl Lee. I printed a copy, reviewed, and PE 2546 SAEED <br /> 12 min attached to the report. I replied to email me the final copy when they receive them. SC 544 <br /> Page 2 of 3 <br />
