Manifest numbers 004202583JJK (5/09) were found on site at the facility but were not on record with the Department of Toxic Substances Control (DTSC). Within 30 days of each shipment, a generator shall submit a legible copy of each manifest used to DTSC. Immediately submit a copy of these manifests to DTSC and ensure that copies of all future manifests are submitted to DTSC within the required time frame. 60, 61, & 62. Manifest numbers 003731392FLE, 007288797JJK (1/25/11), and 007288798JJK (1/25/11) were found without a signed off copy from the destination facility. Copies of manifest numbers 002128762FLE, 007288520JJK, and 007288525JJK were not found on site. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those efforts. Exception reports shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC. By June 24, 2011, submit to the EHD a copy of the signed off copy of the manifests or a copy of the exception report submitted to DTSC. REPEAT VIOLATION 60 & 63. Copies of hazardous waste disposal records for the oil/water separator were not found on site. Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts used in a consolidated manifesting procedure on site for three years and have them readily available for review. Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit copies to the EHD by June 24, 2011. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122).
