Jack Tone Rd., Ripon Program: HW <br /> SUMMARY OF VIOLATIONS <br /> (CLASS 1,CLASS II,or MINOR-Notice to Comply) <br /> 85. Copies of universal waste disposal records were not found on site. A generator of universal waste <br /> shall keep a record of each shipment of universal waste on site for at least three years. The record may <br /> take the form of a log, invoice, manifest, bill of lading, or other shipping document, and shall include the <br /> following: <br /> - Name & address of destination facility <br /> - Quantity & type of universal waste <br /> - Date of departure/shipment <br /> REPEAT VIOLATION <br /> Complete and submit a copy of the Return to Compliance Certification to the EHD by June 24, 2011 <br /> along with a statement documenting the corrective actions that have been or will be taken for each <br /> violation and any supporting paperwork. <br /> **Unable to calculate the amount of hazardous waste generated in 2010 due to missing disposal <br /> records. The 2010 disposal manifests found on site total 1.25 tons of hazardous waste. According to <br /> the general manager, the oil/water separator was cleaned out in 2010- Previous disposal of the oil/water <br /> separator waste was 1500 gallons. The facility will be moved to a lower permit category (5-25 tons/ <br /> year). This may result in decreased permit fees. <br /> Note: (corrected on site) <br /> An empty container which previously held hazardous waste was observed without an empty date. <br /> Containers larger than five gallons that previously held a hazardous material shall be marked with the <br /> date they were emptied and managed within one year of being emptied. The container was labeled and <br /> dated at the time of inspection. <br /> Provided: Return to Compliance Certification form <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> Inspector: <br /> / Rece Title: <br /> EHD Ins <br /> P _ <br /> EHD 23- -003 e/08/1 0/10 CONTINUATION FORM <br />
