Jack Tone Rd. <br /> Ripon, CA 95366 <br /> Dear Ms. Manzo: <br /> This letter is related to issues noted on the Small Quantity Hazardous Waste Generator Inspection Report <br /> completed by the San Joaquin County Environmental Health Department (EHD) during the facility's <br /> annual compliance testing on May 14-15, 2015. Please find below a list of the violations noted at the <br /> referenced facility and Pilot Flying J's (PFJ) responses. <br /> 1. No emergency coordinator. <br /> An emergency coordinator is lacking. There must be at least one emergency coordinator on <br /> site or on call to coordinate emergency response measures, and the following information <br /> must be posted by a phone: the name and phone number of the emergency coordinator; <br /> location of fire extinguishers, spill control equipment, and if present, fire alarm; and the phone <br /> number of the fire department, unless the facility has a direct alarm. A form was provided <br /> that can be used for this purpose. This was corrected onsite. This is a repeat violation noted <br /> on May 23, 2012 inspection report and to date no RTC documentation was received for this <br /> violation. <br /> This is a Class II violation. <br /> Following the inspection on May 23, 2092, a response was provided to the EHD to address <br /> this issue. The Hazardous Materials Management Plan onsite indicated the correct person <br /> (with the correct contact information) as the primary responsible person in the event of an <br /> environmental or hazardous materials emergency. On August 3, 2012, I received a call from <br /> Mr. Muniappa Naidu, REHS, regarding this issue. Although the General Manager was listed <br /> as the emergency contact in the Hazardous Material Management Plan in multiple sections, <br /> the term "Emergency Coordinator" was not used. Mr. Naidu indicated that a page could be <br /> added to the Hazardous Materials Management Plan stating that Brian Cairns is the <br /> Emergency Coordinator for the facility. This was sent to the store and to Mr. Naidu with a <br /> Return to Compliance form ori August 3, 2012, and a copy is enclosed for your review.
