The generator and waste sections of the uniform manifest shall be <br /> completed and signed and dated by the generator. The transporter shall also sign and date <br /> the manifest. Ensure that all manifests are properly completed for any transport of a <br /> hazardous waste for off-site transfer, treatment, storage or disposal. <br /> This-is a minor violation. <br /> California Hazardous Services picks up hazardous waste from all Pilot Flying J facilities on a <br /> quarterly basis. The manifest is left onsite by the transporter at the time of pick up. For <br /> tracking purposes, the corporate office receives the completed manifest with information from <br /> the designated facility. A copy is made for the file and the manifest is sent to the site. <br /> Manifest 008369454JJK has been located, but it is for waste removed from another PFJ <br /> facility. This manifest must have inadvertently been sent to Flying J #618. A copy is <br /> enclosed for your review, and the store has been instructed to send this back to the corporate <br /> office so it can be sent to the correct facility. <br /> 3. Failed to comply with uniform hazardous waste manifest exception requirements. <br /> Manifest 008369454JJK utilized an incorrect EPA ID Number of CAL000354747 instead of <br /> CAL000354766. Submit an exception report to the DTSC to correct this error. <br /> This information shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately submit the required information to DTSC. Submit to the EHD a copy of the <br /> information submitted to the DTSC. <br /> Please refer to#2 above. <br /> 4. Failed to submit a written response within 30 days of receiving an inspection report. <br /> An inspection was last done on May 23, 2012 and an inspection report was issued identifying <br /> information to be submitted to bring this site into compliance. This information was required <br /> to be submitted by June 23, 2014. Only partial information was received resulting in a non- <br /> compliant status for this facility. An operator that receives an inspection report shall have 30 <br /> days to submit a written response that includes a statement documenting corrective actions <br /> taken or proposing corrective actions which will be taken. Ensure that a written response <br /> documenting corrective actions taken or proposed is submitted within 30 days of receiving an <br /> inspection report. <br /> This is a Class II violation. <br /> As previously stated, a response was submitted to the EHD following the Inspection in May <br /> 2012. This response, as well as documentation provided, was meant to address and correct <br /> the violations noted during the inspection. When Mr. Naidu contacted me for more <br /> information, this was provided immediately and Pilot considered the issue resolved and <br /> corrected. <br />
