Furthermore,the service technician on site stated that the 91 product and 87 <br /> product tanks did not have mechanical means of overfill nor was there an audible and visual alarm to alert the fuel <br /> delivery truck driver when the tank has reach above 90%capacity.The service technician on site stated that all <br /> three tanks failed overfill prevention equipment inspection.All underground storage tanks shall be equipped with an <br /> overfill prevention system that does not allow for manual override and meets one of the following requirements: <br /> 1. Alert the transfer operator when the tank is 90 percent full by restricting the flow into the tank or triggering an <br /> audible and visual alarm; or <br /> 2. Restrict delivery of flow to the tank at least 30 minutes before the tank overfills, provided the restriction occurs <br /> when the tank is filled to no more than 95 percent of capacity;and activate an audible alarm at least five minutes <br /> before the tank overfills; or <br /> 3. Provide positive shut-off of flow to the tank when the tank is filled to no more than 95 percent of capacity; or, <br /> 4. Provide positive shut-off of flow to the tank so that none of the fittings located on the top of the tank are <br /> exposed to product due to overfilling. <br /> Immediately contact a properly licensed,trained, and certified contractor to install an approved overfill prevention <br /> system. Replacement of a mechanical flapper valve requires a permit and inspection by the EHD. If an external <br /> audio/visual alarm is replaced, provide a copy of the service receipt to the EHD. If an approved overfill prevention <br /> system can't be installed immediately,there is a possibility that the the 87 product, 91 product and Diesel product <br /> UST system may be red tagged to prevent fuel inputs. <br /> This is a Class I violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by January 27th, 2019. <br /> Document reviewed: Monitoring system certification test results from 2016, 2017, Secondary containment 2018, <br /> employee training, response plan, maintenance records, monitoring plan, financial responsibility documents and <br /> designated operator monthly inspection reports from December 2018-November 2018. <br /> Documents provided: Return to compliance certification. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
