Guild Ave. Lodi, CA 95240 <br />Program: HW <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS II, or MINOR -Notice to Comply) <br />used in a consolidated manifesting procedure on site for three years and have them readily available for <br />review. Immediately locate a copy of all manifests and receipts for the last three years, maintain them <br />on site, and submit copies to the EHD by 6-13-11. Minor -Notice to Comply. <br />79. In the screen prep room, the box of universal waste lamps was not closed. During the inspection, the <br />employee sealed the box of universal waste lamps closed with tape. In the expansion room, the box of <br />universal waste lamps was not closed. A universal waste handler shall contain any lamp in a container <br />or package that is structurally sound, adequate to prevent breakage, and compatible with the contents of <br />the lamp. Such a container or package shall remain closed and shall lack evidence of leakage, spillage <br />or damage that could cause leakage under reasonably foreseeable conditions. Correct immediately by <br />keeping the box of universal waste lamps closed unless adding or removing universal waste lamps. <br />Minor -Notice to Comply. <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, by 6-13-11. <br />Note: <br />3.3 tons of hazardous waste generated in 2010. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: AReceived <br />By: <br />Title: <br />, <br />EHD 23-02-003 Rev 08/10/10 CONTINUATION FORM <br />
