MONITORING AND REPORTI*ROGRAM NO. R5-2010-R5010 • -4 - <br /> PIPELINE <br /> 4 - <br /> PIPELINE CONSTRUCTION DEWATERING <br /> CITY OF STOCKTON <br /> SAN JOAQUIN COUNTY <br /> 1. A statement of the dewatering related activity performed during the monitoring period. If <br /> no groundwater dewatering related activities were performed during the monitoring <br /> period, no further reporting for that month is required. In months when dewatering <br /> activity was performed, the following shall be reported: <br /> a. Results of effluent, pond, sediment disposal, land application area, and groundwater <br /> monitoring. Include any characterization of sediment required by the disposal <br /> facility. <br /> b. A comparison of monitoring data to the discharge specifications, disclosure of any <br /> violations of the NOA and/or WQO, and an explanation of any violation of those <br /> requirements. Data shall be presented in tabular format. <br /> c. Copies of current calibration logs for all field test instruments. <br /> d. Copies of any laboratory analytical report(s) used to characterize water or sediment <br /> quality. <br /> A letter transmitting the self-monitoring reports shall accompany each report. The letter <br /> shall include a discussion of requirement violations found during the reporting period, and <br /> actions taken or planned for correcting noted violations, such as operation or facility <br /> modifications. If the Discharger has previously submitted a report describing corrective <br /> actions and/or a time schedule for implementing the corrective actions, reference to the <br /> previous correspondence will be satisfactory. The transmittal letter shall contain the <br /> following penalty of perjury statement and shall be signed by the Discharger or the <br /> Discharger's authorized agent: <br /> "1 certify under penalty of law that 1 have personally examined and am familiar with <br /> the information submitted in this document and all attachments and that, based on <br /> my inquiry of the those individuals immediately responsible for obtaining the <br /> information, 1 believe that the information is true, accurate, and complete. / am <br /> aware that there are significant penalties for submitting false information, including <br /> the possibility of fine and imprisonment." <br /> The Discharger shall implement the above monitoring program as of the date of this Order. <br /> Ordered by: <br /> /l IJ <br /> PAMELA C.
