Item #209 — Designated operator failed to document Q3 PLLD Shutdown Alarm and check for <br /> appropriate response on the 4/26/2018 designated operator report. <br /> The designated operator had made an error on the designated operator report. On 1/22/2019, <br /> the designated operator who was present at the Monitoring System Certification made the <br /> proper correction to the 4/26/2018 designated operator report, see attached. <br /> Item #304 — Monitoring system not maintained to detect entry into secondary containment, <br /> and <br /> Item#305—The S10 smart sensor did not go into alarm when tested. <br /> On 1/22/2019, Gettler-Ryan technicians replaced the S10 smart sensor and retested with <br /> passing results. This item was corrected at the time of inspection. <br /> Item#306—The 91 STP sensor was not positioned to detect a leak at the earliest opportunity. <br /> - This sensor was in the same position during inspector Betty Ho's inspection on 2/1/2018, <br /> however Betty did not cite this as a violation. <br /> - On 1/24/2019, Getter-Ryan technicians relocated the sensor closer to the sump wall, see <br /> attached pictures as evidence of resolution. <br /> Item#308—The 87 STP liquid sensor failed when tested. <br /> On 1/22/2019, Gettler-Ryan technicians handled the sensor (it was stuck) and retested with <br /> passing results. This item was corrected at the time of inspection. <br /> Item #316 — Clamps for shroud boots on 87 Quad fill and 91 and Diesel fill spill containers <br /> were missing. <br /> On 2/21/2019, Gettler-Ryan replaced all shroud boots and clamps on all 87 Quad fill and 91 and <br /> Diesel fill spill containers, see attached pictures as evidence of resolution. <br /> It is our policy to operate in full compliance with all laws and regulations at our retail facilities. <br /> However, when we fall short of perfection, we endeavor to immediately address deficiencies, issues of <br /> concern, and/or agency requests regarding our retail facilities. With the submittal of this letter, it is our <br /> understanding that the item(s) of alleged non-compliance have been resolved and no further corrective <br /> action is required. Furthermore, we understand that there is no fine or penalty associated with this <br /> violation. If this is not correct, please contact me as soon as possible.
