Aurora Street <br /> Stockton, CA <br /> San Joaquin County Public Health Services, Environmental Health Division (PHS/EHD) <br /> has received a copy of a letter to you from the Central Valley Regional Water Quality <br /> Control Board requesting a work plan to investigate the former location of underground <br /> storage tanks for contamination that propses a minimum of two soil borings. <br /> San Joaquin County has a well ordinance and well standards which are enforced by <br /> PHS/EHD. A well permit is required prior to digging, drilling, boring, driving, repairing or <br /> destroying any well whether it is used for domestic, irrigation, testing, geophysical, <br /> seismic, subsurface boring, monitoring, injection, extraction, vapor probe, cathodic <br /> protection or other purposes. PHS/EHD has to charge for staff time required to approve <br /> well permits and inspect well grouts. The current hourly rate is $89 per hour. <br /> Enclosed is a "Master File Record" form that is used for PHS/EHD billing purposes to <br /> cover the hourly rate for staff time spent on your site. This form must be completed and <br /> submitted to PHS/EHD before we will evaluate or approve any required well permits for <br /> work proposed at your site. In addition, the C-57 licensed driller must obtain a well <br /> permit from PHS/EHD for installation of the borings. The permit fee is $89.
