The terminal automation <br /> system has been preset to load a maximum value of 6000 gallons. <br /> APSA Inspection Report Item #706 - Failed to provide and maintain adequate <br /> secondary containment <br /> The tank referred to as tank#55 was described in the SPCC plan as having insufficient <br /> secondary containment. The SPCC plan and the Addendum Attachment certified by a <br /> professional engineer on 02/15/2017 stated that the secondary containment for the tank <br /> #55 was insufficient. Tank#55 was added in 2013 per the SPCC plan. All bulk storage <br /> tanks must be provided with a secondary means of containment for the entire capacity <br /> of the tank and sufficient freeboard to contain precipitation. Immediately provide <br /> sufficient secondary containment for this and all other tanks at this facility. <br /> The construction of new Storage Tank 32 necessitated the modification of the <br /> secondary containment wall to accommodate the new tank. The modified <br /> secondary containment was completed in August 2018. Updated secondary <br /> containment volumes will be incorporated into the upcoming SPCC plan update <br /> by March 2019. <br /> APSA Inspection Report Item #710 - Plan failed to adequatel cflscuss procedures to <br /> test or inspect each container for integrity <br /> The qualifications of personnel performing formal tests and inspections, frequency and <br /> type of formal testing and inspections required by industry standards are not addressed <br /> in the SPCC plan. The SPCC plan states that in-service inspections are conducted to <br /> API industry standard and that Tesoro has established testing schedule to assure that <br /> aboveground storage tanks are tested in accordance with existing industrial and <br /> regulatory standards. The testing schedule and inspector qualifications per industry <br /> standards are not discussed. Each aboveground container shall be tested and <br /> inspected for integrity on a regular schedule and whenever repairs are made. The <br /> qualifications of personnel performing tests and inspections, frequency and type of <br /> testing and inspections that take into account container size, configuration, and design <br /> shall be determined in accordance with industry standards. Examples of these integrity <br /> tests include, but are not limited to: visual inspection, hydrostatic testing, radiographic <br /> testing, ultrasonic testing, acoustic emissions testing, or other systems of non- <br /> destructive testing. Comparison records and other records of inspections and tests <br /> must be maintained on site. Immediately conduct the necessary testing and submit a <br /> copy of the test results to the EHD, or provide equivalence as allowed by CFR <br /> 112.7(a)(2) and amend the SPCC plan to discuss testing and inspection frequencies <br /> and qualifications of personnel performing the tests and inspections, per industry <br /> standards. <br /> Tesoro Logistics maintains a tank inspection program to ensure the structural <br /> and mechanical integrity of its storage tanks. Routine daily, monthly, quarterly, <br /> annual, and other periodic inspections and testing are conducted by qualified and <br /> trained terminal operators, internal company inspectors, and contracted <br />
