SANJOAQUIN Environmental Health Department <br /> --COU N l Y <br /> Complaint Record Daily Time and Activity Summary from 1/3/2018 to <br /> 1/3/2019 for COMBPSGNB located at <br /> STOCKTON GOLF & COUNTRY CLUB, 3800 W. <br /> COUNTRY CLUB BLVD. STOCKTON, CA 95204 <br /> Activity Date Recorded by <br /> 08/07/2018 12:30 PM - Received complaint from Muniappa Naidu, who received a call from PE 2546 FLORIDO <br /> 150 min Stockton Fire Dept. reporting an unknown type and quantity of chemical inside a SC 544 <br /> trash compactor outside of the kitchen at Stockton Golf& Country Club, causing <br /> fumes to be released from the contractor. <br /> 1 PM -Arrived on site with Kashi Ali and met with Stockton Fire Dept. Stockton Fire <br /> cautioned off the area containing the trash compactor located in an open outside <br /> area, about 20 feet from the kitchen back door. Stockton Fire had diluted the <br /> unknown chemicals inside compactor with water to prevent a fire. No fumes, <br /> vapors, or spillage was observed and no chemical smell was sensed near the the <br /> compactor at the time of inspection. According to the General Manager, Rich <br /> Howarth, and the head chef, a strong ammonia or bleach-like odor was smelled <br /> from inside the kitchen and white fumes were observed coming from the trash <br /> compactor. The back kitchen door was closed immediately and an industrial fan <br /> was set up inside the kitchen to get rid of the odor from inside. There was no <br /> chemical odor or other signs of contamination inside the kitchen at the time of <br /> inspection. The food prep areas in the kitchen have been properly sanitized and all <br /> food that was out at the time of the incident has been disposed of. Issued Notice <br /> to Abate to Rich Howarth. Left site. <br /> Informed John Alaniz of the spill and that he may need to verify clean-up <br /> afterhours. I called Rich who indicated Patriot's plan to clean the trash compactor <br /> today, take a sample of the spilled chemical for analytical, and leave the waste on <br /> site until profiled for disposal. Rich provided the contact information for the Patriot <br /> Environmental foreman onsite, Nathan Baker(209-829-2198). <br /> 08/08/2018 Discussed complaint with Elianna and Garrett. Completed daily from that day. PE 2546 ALANIZ <br /> 42 min SC 544 <br /> 08/09/2018 Downloaded photos. Created photo log and emailed it to Elianna. PE 2546 ALANIZ <br /> 18 min SC 544 <br /> 01/02/2019 Reviewed narrative. Emailed Rich Howarth (rhowarth@stocktongolfcc.com) the PE 2546 FLORIDO <br /> 12 min following: "This email is to notify you that I have not received the sample analytical, SC 544 <br /> nor the disposal records, for the waste generated from the trash compactor <br /> chemical spill clean-up at Stockton Golf& Country Club on 8/7/18. <br /> Two 55-gallon drums of solid waste and two 55-gallon drums of liquid waste were <br /> generated from the clean-up done by Patriot Environmental. <br /> Submit the sample analytical and disposal records for the waste as soon as <br /> possible. <br /> Please note all time spent abating this complaint will be billed at our current rate of <br /> $152/hour. <br /> Feel free to contact me if you have any questions." <br /> 01/03/2019 Reviewed received invoices and hazardous waste manifest from Rich Howarth of PE 2546 FLORIDO <br /> 24 min Stockton Country Club. Printed John Alaniz's, REHS, photo log of the spill SC 544 <br /> clean-up. Generated photo log with my inspection photos. Generated Emergency <br /> Response Record Form. Generated the Prop 65 and provided to Garrett Backus, <br /> REHS, for his review. Gathered complaint documentation for abatement. <br /> Page 2 of 2 <br />
