New West Petroleum #1003 • - 2 - • 21 December 2012 <br /> 6437 Banner Street, Lodi <br /> San Joaquin County <br /> On 30 April 2007, the Executive Officer of the Central Valley Regional Water Quality Control <br /> Board (Central Valley Water Board) issued Cleanup and Abatement Order R5-2007-0709 (CAO) <br /> to J. Gilbert Moore, Ms. Eileen A. Moore, and New West Stations, Inc. for 6437 Banner Avenue <br /> (AKA Banner Street) in Lodi (Site). All tasks as required by the CAO have been successfully <br /> completed. And all monitoring wells have been properly destroyed under permit from the <br /> San Joaquin County Environmental Health Department. The Site has been restored through <br /> removal and/or deactivation of all groundwater treatment equipment. Therefore, the CAO is <br /> hereby rescinded. <br /> Please contact Mr. James Barton by phone at (916) 464-4615, or via e-mail at <br /> ibarton(a-)waterboards.ca.gov if you have any questions regarding this matter. <br /> PAMELA C.
