Banner Avenue
Lodi, CA. 95242
Dear Mr. Moore:
The Central Valley Regional Water Quality Control Board (CVRWQCB) has reviewed and
approved Monitoring Well Destruction Work Plan (WP), dated 24 July 2012, for the proper
destruction of 42 monitoring and remediation wells installed as part of the corrective action
taken to address the unauthorized release of petroleum hydrocarbons from your underground
storage tank (UST) system. The WP was prepared by your consultant, Advanced
GeoEnvironmental Inc. (AGE).
As the permitting agency, the San Joaquin County Environmental Health Department(EHD) has
also reviewed WP for compliance with the San Joaquin County Well Standards. The EHD has
found the proposed well destruction methods in WP to comply with the Well Standards and also
approves destruction of the wells as proposed in WP. Any deviation from the methods proposed
in the WP must be approved by the EHD. Please submit the necessary permit applications and
associated fees to the EHD.
If you have any comments and/or questions, please contact Nuel Henderson at the address
noted on this correspondence or call him at (209)468-3436.
Sincerely,
Nuel Henderson, PG Adrienne Ellsaesser
Engineering Geologist Acting Program Coordinator
c: CVRWQCB—James Barton, Sacramento
c: SWRCB-CUF—Mark Owens, Sacramento
c: AGE—Timothy Cuellar
Well Destruction Work Plan Approval Letter 0812
