MONITORING AND REPORTINAOGRAM NO. R5-2008-0819 -4 - PIONEER 4 - PIONEER AMERICAS LLC TRACY, SAN JOAQUIN COUNTY REPORTING Groundwater monitoring and sampling data shall be submitted to the California State Water Resources Control Board's GeoTracker database system and written reports shall be submitted to the Regional Water Board. When reporting the data, the Discharger shall arrange the information in tabular form so that the date, the constituents, and the concentrations are readily discernible. The data shall be summarized in such a manner as to illustrate clearly the compliance with this Order. In addition, the Discharger shall notify the Regional Water Board verbally within 48 hours of any unscheduled shutdown of any wellhead treatment system(s). As required by the California Business and Professions Code Sections 6735, 7835, and 7835.1, all reports shall be prepared by a registered professional or their subordinate and signed by the registered professional. The Discharger shall submit a report on the first and second quarter monitoring events to the Regional Water Board by 1 August of each year until such time as the Executive Officer determines that the report is no longer necessary. The report shall include the following minimum information from the first and second quarter monitoring and sampling events: (a) A statement regarding the first and second quarter groundwater monitoring and sampling events; (b) A table or tables of the piezometric surface measurements and laboratory chemical analyses results for the first and second quarter monitoring and sampling events; (c) A table of any wellhead treatment system(s) sample analyses results from the first six months of the year; (d) Field logs that contain, at a minimum, water quality parameters measured before, during, and after purging, depth to water before and after purging, method of purging, volume of water purged, and time of purging and sampling; (e) Groundwater piezometric surface contour maps of the Upper, Middle, and Lower Horizons for the first quarter event; (f) A table showing construction details of all wells including well number, groundwater horizon monitored, wellhead reference elevation, depth to bottom of well, depth to bottom of borehole, and depth intervals of the screen, the bentonite seal, and the filter pack; (g) Copies of the laboratory chemical analyses reports from the first and second quarters; (h) Status of any wellhead treatment system(s), including operating time, effectiveness, and any field notes pertaining to the operation and maintenance of the system(s); and (i) The reasons for, and duration of all interruptions in the operation of wellhead treatment system(s), and actions planned or taken to correct and prevent future interruptions. The Discharger shall submit an Annual Report to the Regional Water Board by 1 February of each year until such time as the Executive Officer determines that the report is no longer necessary. This report shall contain information from the third and fourth quarter monitoring and sampling events and evaluations of the effectiveness and progress of the investigation, remediation, and any wellhead treatment system(s). The Annual Report shall contain the following minimum information:
