MONITORING AND REPORTINAOGRAM NO. R5-2008-0819 -4 - <br /> PIONEER <br /> 4 - <br /> PIONEER AMERICAS LLC <br /> TRACY, SAN JOAQUIN COUNTY <br /> REPORTING <br /> Groundwater monitoring and sampling data shall be submitted to the California State Water <br /> Resources Control Board's GeoTracker database system and written reports shall be <br /> submitted to the Regional Water Board. When reporting the data, the Discharger shall <br /> arrange the information in tabular form so that the date, the constituents, and the <br /> concentrations are readily discernible. The data shall be summarized in such a manner as to <br /> illustrate clearly the compliance with this Order. In addition, the Discharger shall notify the <br /> Regional Water Board verbally within 48 hours of any unscheduled shutdown of any <br /> wellhead treatment system(s). <br /> As required by the California Business and Professions Code Sections 6735, 7835, and <br /> 7835.1, all reports shall be prepared by a registered professional or their subordinate and <br /> signed by the registered professional. <br /> The Discharger shall submit a report on the first and second quarter monitoring events to the <br /> Regional Water Board by 1 August of each year until such time as the Executive Officer <br /> determines that the report is no longer necessary. The report shall include the following <br /> minimum information from the first and second quarter monitoring and sampling events: <br /> (a) A statement regarding the first and second quarter groundwater monitoring and sampling <br /> events; <br /> (b) A table or tables of the piezometric surface measurements and laboratory chemical <br /> analyses results for the first and second quarter monitoring and sampling events; <br /> (c) A table of any wellhead treatment system(s) sample analyses results from the first six <br /> months of the year; <br /> (d) Field logs that contain, at a minimum, water quality parameters measured before, during, <br /> and after purging, depth to water before and after purging, method of purging, volume of <br /> water purged, and time of purging and sampling; <br /> (e) Groundwater piezometric surface contour maps of the Upper, Middle, and Lower <br /> Horizons for the first quarter event; <br /> (f) A table showing construction details of all wells including well number, groundwater <br /> horizon monitored, wellhead reference elevation, depth to bottom of well, depth to bottom <br /> of borehole, and depth intervals of the screen, the bentonite seal, and the filter pack; <br /> (g) Copies of the laboratory chemical analyses reports from the first and second quarters; <br /> (h) Status of any wellhead treatment system(s), including operating time, effectiveness, and <br /> any field notes pertaining to the operation and maintenance of the system(s); and <br /> (i) The reasons for, and duration of all interruptions in the operation of wellhead treatment <br /> system(s), and actions planned or taken to correct and prevent future interruptions. <br /> The Discharger shall submit an Annual Report to the Regional Water Board by 1 February of <br /> each year until such time as the Executive Officer determines that the report is no longer <br /> necessary. This report shall contain information from the third and fourth quarter monitoring <br /> and sampling events and evaluations of the effectiveness and progress of the investigation, <br /> remediation, and any wellhead treatment system(s). The Annual Report shall contain the <br /> following minimum information: <br />
