MONITORING AND REPORTIWROGRAM NO. R5-2008-0819 - 5 - PIONEER 5 - PIONEER AMERICAS LLC TRACY, SAN JOAQUIN COUNTY (a) Descriptions of all groundwater monitoring and sampling events conducted during the year; (b) A table showing construction details of all wells including well number, groundwater horizon monitored, wellhead reference elevation, depth to bottom of well, depth to bottom of borehole, and depth intervals of the screen, the bentonite seal, and the filter pack; (c) A table or tables of all piezometric surface measurements and laboratory chemical analyses results collected during the year; (d) A table of all wellhead treatment system(s) sample analyses results collected during the year from any operating systems; (e) Cumulative data tables containing all previous water quality analytical results and depths to groundwater collected from all existing and former groundwater monitoring and water supply wells; (f) Table(s) showing lateral and vertical flow directions and gradients during each quarter of the year; (g) Graphical summaries of pertinent data obtained during the year; (h) Field logs that contain, at a minimum, all water quality parameters measured before, during, and after purging, depth to water before and after purging, method of purging, volume of water purged, and time of purging and sampling; (i) Copies of all laboratory chemical analyses reports prepared during the third and fourth quarters of the year; (j) Groundwater piezometric surface contour maps of the Upper, Middle, and Lower Horizons for the third quarter event; (k) Pollutant concentration maps containing all carbon tetrachloride and chloroform data obtained during the year; (1) A discussion of the long-term trends in the concentrations of the pollutants in the groundwater monitoring wells and an analysis of whether the pollutant plumes are shrinking, stable, or are spreading; (m)Status of any wellhead treatment system(s), including operating times, effectiveness, and any field notes pertaining to the operation and maintenance of the system(s); (n) The reasons for, and duration of all interruptions in the operation of any wellhead treatment system(s), and actions planned or taken to correct and prevent future interruptions. (o) The anticipated date for completion of cleanup activities; (p) An identification of any data gaps and potential deficiencies/redundancies in the monitoring system or reporting program; and (q) If desired, a proposal and rationale for any revisions to the groundwater sampling plan frequency and/or 'list of analytes. The Discharger shall submit electronic data reports, which conform to the requirements of the California Code of Regulations, Title 23, Division 3, Chapter 30. The reports shall be submitted electronically over the Internet to the GeoTracker database system by 1 February of each year for the annual event and 1 August of each year for the semi-annual event until such time as the Executive Officer determines that the reports are no longer necessary.
