R5-2008-0819 - 5 - <br /> PIONEER <br /> 5 - <br /> PIONEER AMERICAS LLC <br /> TRACY, SAN JOAQUIN COUNTY <br /> (a) Descriptions of all groundwater monitoring and sampling events conducted during the <br /> year; <br /> (b) A table showing construction details of all wells including well number, groundwater <br /> horizon monitored, wellhead reference elevation, depth to bottom of well, depth to bottom <br /> of borehole, and depth intervals of the screen, the bentonite seal, and the filter pack; <br /> (c) A table or tables of all piezometric surface measurements and laboratory chemical <br /> analyses results collected during the year; <br /> (d) A table of all wellhead treatment system(s) sample analyses results collected during the <br /> year from any operating systems; <br /> (e) Cumulative data tables containing all previous water quality analytical results and depths <br /> to groundwater collected from all existing and former groundwater monitoring and water <br /> supply wells; <br /> (f) Table(s) showing lateral and vertical flow directions and gradients during each quarter of <br /> the year; <br /> (g) Graphical summaries of pertinent data obtained during the year; <br /> (h) Field logs that contain, at a minimum, all water quality parameters measured before, <br /> during, and after purging, depth to water before and after purging, method of purging, <br /> volume of water purged, and time of purging and sampling; <br /> (i) Copies of all laboratory chemical analyses reports prepared during the third and fourth <br /> quarters of the year; <br /> (j) Groundwater piezometric surface contour maps of the Upper, Middle, and Lower <br /> Horizons for the third quarter event; <br /> (k) Pollutant concentration maps containing all carbon tetrachloride and chloroform data <br /> obtained during the year; <br /> (1) A discussion of the long-term trends in the concentrations of the pollutants in the <br /> groundwater monitoring wells and an analysis of whether the pollutant plumes are <br /> shrinking, stable, or are spreading; <br /> (m)Status of any wellhead treatment system(s), including operating times, effectiveness, and <br /> any field notes pertaining to the operation and maintenance of the system(s); <br /> (n) The reasons for, and duration of all interruptions in the operation of any wellhead <br /> treatment system(s), and actions planned or taken to correct and prevent future <br /> interruptions. <br /> (o) The anticipated date for completion of cleanup activities; <br /> (p) An identification of any data gaps and potential deficiencies/redundancies in the <br /> monitoring system or reporting program; and <br /> (q) If desired, a proposal and rationale for any revisions to the groundwater sampling plan <br /> frequency and/or 'list of analytes. <br /> The Discharger shall submit electronic data reports, which conform to the requirements of <br /> the California Code of Regulations, Title 23, Division 3, Chapter 30. The reports shall be <br /> submitted electronically over the Internet to the GeoTracker database system by 1 February <br /> of each year for the annual event and 1 August of each year for the semi-annual event until <br /> such time as the Executive Officer determines that the reports are no longer necessary. <br />
