MONITORING AND REPORTIOROGRAM NO. R5-2008-0819 - 6 - <br /> PIONEER <br /> 6 - <br /> PIONEER AMERICAS LLC <br /> TRACY, SAN JOAQUIN COUNTY <br /> In the event that any wellhead treatment system(s) are installed and operated in the furture, <br /> the Discharger shall submit monthly wellhead treatment system monitoring data reports to <br /> the Regional Water Board by the 28th day of the following month until such time as the <br /> Executive Officer determines that the report is no longer necessary. Each monthly report <br /> shall include the following minimum information: <br /> (a) A table or tables of the wellhead treatment system(s) influent, midstream, and effluent <br /> water sample laboratory chemical analyses data collected during the month; <br /> (b) Status of the wellhead treatment system(s), including operating time, effectiveness, and <br /> any field notes pertaining to the operation and maintenance of the system(s); <br /> (c) The reasons for, and duration of all interruptions in the operation of the wellhead <br /> treatment system(s) and actions planned or taken to correct and prevent future <br /> interruptions; and <br /> (d) Copies of the laboratory chemical analyses reports for all samples collected from all <br /> systems during the month. <br /> The results of any monitoring done more frequently than required at the locations specified in <br /> the MRP also shall be reported to the Regional Water Board.
