There must be at least one <br /> emergency coordinator on site or on call to coordinate emergency response measures, and the following information <br /> must be posted by a phone: the name and phone number of the emergency coordinator; location of fire extinguishers, <br /> spill control equipment, and if present, fire alarm; and the phone number of the fire department, unless the facility has <br /> a direct alarm. Immediately appoint an emergency coordinator and post the required information by a phone. A form <br /> is provided that can be used for this purpose. <br /> This is a minor violation. <br /> 117 HSC 25404(e)(4) Failed to report program data electronically. <br /> A submission to the California Electronic Reporting System (CERS)for the hazardous waste program has not been <br /> made. Beginning January 1, 2013, all businesses are required to submit all new(or any changes to existing) <br /> hazardous waste information online to the CERS at http://cers.calepa.ca.gov. Be sure to include your hazardous <br /> waste activity in the Businesses Activities section in CERS in addition to any other relevant activities and required <br /> fields. <br /> This is a Class II violation. <br /> 301 CCR 66262.34(d)(2) Facility not maintained to minimize the release of a hazardous waste. <br /> Oily staining and pooled oil were noted inside the secondary containment and on the floor around one oil filled piece of <br /> equipment located inside the chemical storage area. Furthermore, white crystal deposits and floor corrosion were <br /> noted around a large tank also located in the chemical storage area. Facilities shall be maintained and operated to <br /> minimize the possibility of a fire, explosion, or release of hazardous waste to air, soil, or surface water which could <br /> threaten human health or the environment. Immediately fix all leaks and staining and manage the recovered waste <br /> according to Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining how <br /> this waste was managed. <br /> This is a minor violation. <br /> 303 CCR 66262.34(d)(2) Failed to maintain all communication or alarm systems, spill control, or decontamination <br /> equipment. <br /> An eye wash station located in the chemical storage area was not maintained. All facility communications or alarm <br /> systems, fire protection equipment, spill control equipment, and decontamination equipment, where required, shall be <br /> tested and maintained as necessary to assure its proper operation in time of emergency.
