Beckman Road <br /> Lodi , CA 95240 <br /> 209-333-2233 <br /> 3) The project manager , and the health and safety coordinator is <br /> Keith A . Tallia of Oil Equipment Service . The site safety <br /> officers at the job site will be Keith A . Tallia of Oil <br /> Equipment Service . The alternate site safety officer will be <br /> James P . Hesketh of Oil Equipment Service . <br /> 4) Oil Equipment Service will have on the site ( 2) fire extin- <br /> quishers and one ( 1) first-aid kit within 50 ' of hole location . <br /> See attached map . No smoking will permitted within 50 ' of <br /> the hole or areas where flammable vapors are present . <br /> 5) Oil Equipment Service shall notify the local fire department <br /> 48 hours before excavation shall begin . USA will also be <br /> notified before excavation o.f job . <br /> 6) Oil Equipment Service will approach all excavations with <br /> possible cave-ins : The following rules will be followed to <br /> prevent any mishaps : 1) soil to be inspected for stability <br /> before excavation begins , 2) soil is to be kept at least two <br /> (2) feet from the edge of an excavation , 3) if excavation is <br /> five (5) feet or more the hole shall be tapered , braced , or <br /> other equivalent methods shall be used to prevent possible <br /> cave-ins . Precautions will be taken at all times . <br /> 7) At the job site , clean fill will be used to backfill the empty <br /> hole after soil samples have been taken by Condor Earth <br /> Technologies Inc . Soil samples will be excavated using backhoe . <br /> Samples will be taken two (2) feet to four (4) feet by hand <br /> using brass tubes . All of the above procedures will be done <br /> according to code . Any part not backfill shall be barricaded <br /> with construction barricades and caution tape will be used . <br /> All sampling to be per Condors workplan . <br />
