S ECOR <br /> Ms. Lori Duncan <br /> February 18, 2005 <br /> Page 2 <br /> I <br /> SITE HEALTH AND SAFETY PLAN <br /> As required by the Occupational Health and Safety Administration (OSHA) "Hazardous <br /> Waste Operations and Emergency Response" guidelines (29 CFR 1910.120), SECOR will <br /> prepare a site specific Health and Safety Plan (HASP). The field staff and contractors will <br /> review the HASP prior to beginning field operations at the site. <br /> WELL DESTRUCTION AND FACILITY REMOVAL ACTIVITIES <br /> Proposed activities include the following: <br /> Permitting <br /> The applicable permits will be obtained from the City of Stockton and San Joaquin County <br /> Environmental Health Division (County), as necessary, prior to drilling. <br /> Underground Utility Locating <br /> Underground Service Alert (USA) will be notified a minimum of 48 hours prior to <br /> commencing field activities to identify any public utility alignments that may be in conflict <br /> with the proposed borings. In conjunction with the USA, a private utility locating company <br /> will be utilized for clearing proposed boring 'locations for underground utilities. <br /> Well Destruction <br /> The wells will be destroyed by drilling out and removing all material within the original <br /> borehole including the well casing, screen, filter pack, and annular seal to the total depth of <br /> the well. The boreholes will then be pressure grouted using a tremie pipe and appropriate <br /> sealing material from the bottom of the borehole to surface. The wells will then be <br /> completed to grade. <br /> Facility Removal and Sampling Activities <br /> SECOR will oversee the removal of the USTs, fuel piping, and fuel dispensers, including <br /> water samples <br /> in accordance with the California <br /> the collection of all required soil and ground les p <br /> Regional Water Quality Control Board's Tri-Regional Board Staff Recommendations for <br /> Preliminary Investigation and Evaluation of Underground Tank Sites, dated August 10, <br /> 1990. In addition to the removal of all underground fuel facilities, SECOR will direct the <br /> excavation contractor to remove observed significantly impacted soil to the extent <br /> practicable.
